CONSENT & RISK POLICY

1. CONSENT TO PARTICIPATION: I/We hereby authorize and consent to our child's participation in Ace Volleyball Club programming.
2. CONSENT TO EMERGENCY MEDICAL CARE/TREATMENT: I/We hereby give our consent and authorize the Ace Volleyball Club and its directors, staff, coaches, volunteers, officers, agents, employees and representatives, (collectively, 'Ace') to act on our behalf and on behalf of our child, to provide emergency medical care and treatment in the event Ace is unable to contact me/us by reasonable attempts in the event of the need for emergency medical care and treatment.
3. INHERENT RISKS: I/We acknowledge that there are risks inherent in the sport of volleyball, including but limited to sprains, strains and fractures, contact with balls, other players, court and player bench equipment, all of which could result in personal injury or even death, property loss and financial loss. I acknowledge that my/our son/ daughter is participating in the Camp at his/her own risk and I/we as well as my/our son/daughter are in full knowledge of the hazards and potential hazards, which are inherent in this sport.
4. COVID 19: Further, I/We, understand that there is a risk of infection from the Covid 19 virus, given the nature of the sport of volleyball, including but not limited to contact with balls, nets and other equipment, proximity with other program participants, their parents and guardians, coaches and staff. I/We will promptly notify Ace Volleyball of any symptoms in my child or my child's immediate family members, including cough, fever, runny nose, headache, chills, fatigue, stomach upset, loss of sense of smell or taste and/or generally feeling unwell.
5. WAIVER OF LIABILITY AND DISCHARGE OF ACE VOLLEYBALL: I/We, the undersigned, for ourselves, our heirs, executors and administrators, waive, release and forever discharge Ace Volleyball Club and its directors, staff, coaches, volunteers, officers, agents, employees, representatives, successors and assigns from any and all responsibility, liability, claims, demands, actions and causes of action of any kind whatsoever, including the negligence of one or more individuals and organizations referred to herein, arising out of or related to any loss, personal injury, illness or property damage that may be sustained or occur during participation in Ace program activities.
6. ACKNOWLEDGEMENT AND ACCEPTANCE: I/We have read this document and fully understand the risks involved in participation in the program. I/We agree to the terms and conditions stated herein, and understand that by signing this document, I/We are giving up certain civil rights, including the right to sue Ace Volleyball Club and its directors, staff, coaches, volunteers, officers, agents, employees, representatives, successors and assigns, even if they are negligent. I agree this Waiver of Liability is binding upon my/our and my son/daughter's executors, heirs and assigns.
