REGISTRATION AGREEMENT
I/we hereby certify that I/we are the parent(s) or legal guardian(s) of the player(s) and that I give my/our consent for him or her to attend an Spring Tryouts within the Airdrie Minor Hockey Association (AMHA).
I/we understand that:
- We are responsible to pay all AMHA tryout registration fees and that my child will not be allowed on the ice with AMHA until all fees are paid to date and documentation have been received by AMHA.
- If we have outstanding fees from a previous season, they must be paid before our player will be allowed on the ice for the spring camps, this includes any outstanding fees, tryouts/evaluations fees and/or volunteer or jersey payments owed.
After you register, if your player changes their mind and no longer want to participate in the spring tryout, please refer to the 7.3 Refund Policy, found under the Information tab - Bylaws, Policies & Procedures - Refund Policies on the AMHA website.
FREEDOM OF INFORMATION POLICY (FOIP)
I understand by acknowledging below that I/we are giving consent and release under the Freedom of Information Policy for posting personal information to any of the Airdrie Minor Hockey Association and its affiliates websites, social media sites, printed and written material.
- The purpose of this notice and consent is to inform you of the use to which your player information, pictures, and game statistics could be used and to obtain your consent for such use.
- The player information is collected and maintained to properly coordinate and operate the Airdrie Minor Hockey program and is also provided to Hockey Alberta, Hockey Canada, and any league the player’s team participates in, for registration, recording statistical information and insurance purposes.
- Player’s names, information, data, photos, and comments may also be used in team or league newsletters, social media posts, annual reports, Airdrie Minor Hockey Association, Hockey Alberta websites, Hockey Canada websites, radio, newspaper and other hockey or local publications.
- Individual and team photos may be taken and displayed in the local newspaper, local arenas, other reports, and advertisements prepared by Airdrie Minor Hockey Association.
- Player’s names and parents/guardians’ telephone, email and other information may be used for the purposes of team, league and for Airdrie Minor hockey association communication, transportation matters and services.
- Player’s names may be included on lists for the purpose of, team placement and for tracking player statistics by the team, league, Airdrie Minor Hockey Association, Hockey Alberta, and such statistics may be displayed on their websites.
- This consent shall remain in full force for as long as the Player is a member of Airdrie Minor Hockey Association and need only be acknowledged once.
RESPECT MATTERS POLICY 2.1
As a condition of AMHA registration and participation, it is mandatory for AMHA members to familiarize themselves with the content of the Respect Matters policy, specifically what behaviour constitutes breaches of the policy and what corrective measures may be taken by AMHA after investigation.
