I hereby certify that I am the parent/guardian of the above named player and give my consent for them to participate in the AYL. I also agree to hold harmless the AYL and officer or trustee, team coach, assistant coach, or any and all volunteer helpers in the program, for any and all injuries, which may be incurred by the above named player.

I also certify by my signature below that I have read and agree to uphold the Ansonia Youth League Code of Conduct as is outlined below--

The Ansonia Youth League is committed to providing its players and opponents with a safe playing environment at all times. The players, parents, guardians, family members and spectators are expected to adhere to and abide by the following Code of Conduct at all times. Behavior and actions which are inconsistent with the Code of Conduct are subject to disciplinary measures at the discretion of the Ansonia Youth League. The Code of Conduct, and the expectations within, will extend to all AYL events, including practices, games, tournaments, etc.

1 - I (and my guests) will treat all players, coaches, parents, umpires, administrators, and opponents with respect and dignity.

2 - I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes.

3 - I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.

4 - I will place the players first and the game second.

5 - I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the outcome of the game or his/her performance.

6 - I will praise my child for competing fairly and trying hard and make my child feel like a winner every time.

7 - I will never ridicule or yell at my child or other participants for making or losing a competition. All of my comments will be verbalized in a positive constructive manner, never emphasizing the negative.

8 - I will learn the rules of the game and policies of the league.

9 - I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.

10 - I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, color, sex or ability.

11 - I will never physically or verbally abuse a player regardless of the situation.

12 - I will never verbally abuse a coach or an umpire regardless of the situation.

13 - I will not tolerate unsportsman-like conduct from my child or guests.

14 - I will refrain from coaching my child or other players during games and practices unless I am one of the official coaches of the team or asked to help by one of the official coaches of the team.

15 - I understand that my child is a member of a “TEAM” and will make sure my child is at practice and games on time and will contact the coach if my child is unable to attend practice or a game.

16 - I will not use tobacco, alcohol or drugs during games and practices. No smoking, use of alcohol or drugs will be tolerated by any coach, player, fan, parent, or umpire during games on the field.

17 - Most importantly, I will always ensure that my child maintains his/her right to have fun.
