Arena District Kickball League – Liability Waiver & Assumption of Risk

In consideration of being permitted to participate in the Arena District Athletic Club (“ADAC”) kickball league and related activities (the “Activity”), I acknowledge, understand, and agree:

1. Assumption of Risk. I understand that participation in the Activity involves inherent risks, including but not limited to slips, falls, collisions, overexertion, and serious injury or death. While rules, equipment, and personal discipline may reduce these risks, they cannot eliminate them.

2. Voluntary Participation. I knowingly and voluntarily assume all risks of participation, whether known or unknown, foreseeable or unforeseeable, including risks that may arise from the negligence of ADAC or others, to the fullest extent permitted by law. I accept full responsibility for my participation.

3. Compliance / Safety. I agree to follow all rules and instructions and to conduct myself responsibly. If I observe any unusual or significant hazard, I will stop participating and notify a league official immediately.

4. Release and Hold Harmless. On behalf of myself and my heirs, assigns, personal representatives, and next of kin, I release and hold harmless ADAC and its officers, directors, officials, agents, employees, volunteers, sponsors, other participants, and the owners/lessors of any premises used for the Activity (collectively, the “Releasees”) from any and all claims or liability for injury, disability, death, or loss or damage to person or property arising out of or related to my participation, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

5. Medical Treatment (optional but recommended). In the event of injury or illness, I authorize ADAC to obtain emergency medical treatment for me, and I agree to be responsible for any related costs.

6. Governing Law / Severability (optional but recommended). This agreement is governed by the laws of the State of Ohio. If any provision is found unenforceable, the remaining provisions will continue in full force and effect.

Electronic Acknowledgment. By checking the box below, I certify that I am at least 18 years old, I have read and understand this Waiver and Assumption of Risk, I understand I am giving up substantial rights, and I agree freely and voluntarily. Checking the box constitutes my electronic signature.
