Waiver:
In consideration of the acceptance of this entry in the Balgonie Soccer & Triangle Area Youth Soccer Associations soccer program, I, for myself, my heirs, executors, administrators, and assignors, waive any claims to which I may become entitled for injury or damage and release the board and all other organizers, sponsors, representatives, their agents, and any other person or organization assisting in the soccer events from any claims for damages or injury suffered by the participant(s) as a result of participation in or travelling to or from events.

Privacy Acknowledgement
I acknowledge that Balgonie Soccer & Triangle Area Youth Soccer Associations collects personal information from its membership:
a) For administrative and operational purposes only; and
b) Uses the information only for advancing the goals of Balgonie Soccer & Triangle Area Youth Soccer Associations and to facilitate communication between Balgonie Soccer & Triangle Area Youth Soccer Associations and its members; and
c) Does not disclose personal information to any third parties for any purpose.
Photography Acknowledgement:
I acknowledge that Balgonie Soccer & Triangle Area Youth Soccer Associations may use photographs or video of participants (myself, my child(ren)) taken during their participation in Balgonie Soccer & Triangle Area Youth Soccer Associations activities. These images will only be used for the purposes of promoting the goals of Balgonie Soccer & Triangle Area Youth Soccer Associations. I understand that myself, or my child, will not be identified in any of these photographs without my permission.
I acknowledge that parents may take photos or videos of their own child(ren) while participating in Balgonie Soccer & Triangle Area Youth Soccer Associations soccer. Photographs or videos of any others involved in Balgonie Soccer & Triangle Area Youth Soccer Associations soccer are not permitted under any circumstance.
CASL Message:
One of Balgonie Soccer & Triangle Area Youth Soccer Associations primary method of communication with players, parents, referees, and coaches is via email. By enrolling in our soccer program you are authorizing Balgonie Soccer & Triangle Area Youth Soccer Associations to email you information related to soccer and the Association's sponsors. This includes announcements, game reminders, practice times, etc. If you wish to opt out of receiving email communications, please do so by written request sent to balgoniesoccer@gmail.com.
