BELTWAY LACROSSE (BLX)
ADULT PARTICIPANT RELEASE, WAIVER OF LIABILITY,
ASSUMPTION OF RISK & INDEMNIFICATION AGREEMENT
(Virginia – Adult / Masters League)
PLEASE READ CAREFULLY.
THIS IS A LEGALLY BINDING DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS.
In consideration of being permitted to participate in any league, game, tournament, clinic, training session, open play, or related event or activity (collectively, the “Program”) operated, organized, or sponsored by Beltway Lacrosse (BLX), the undersigned individual (“Participant”) agrees as follows:
1. Acknowledgment of Risk
Participant understands and acknowledges that lacrosse is a full-contact sport involving sticks, hard balls, body contact, rapid movement, and physical exertion. Participation carries inherent risks, including but not limited to:

Concussions and other head injuries

Fractures, dislocations, sprains, strains, and ligament injuries

Dental and facial injuries
-
Permanent disability, paralysis, or death
Participant acknowledges that these risks may arise from their own actions, the actions of other participants, conditions of the playing surface or facility, equipment failure, or the negligence of others, and that not all risks can be eliminated.
Participant represents and warrants that they are 18 years of age or older, physically capable of participation, and have no medical or physical condition that would prevent safe participation.
2. Assumption of Risk
Participant knowingly, voluntarily, and freely assumes all risks, both known and unknown, foreseeable and unforeseeable, associated with participation in the Program, even if arising from the ordinary negligence of Beltway Lacrosse (BLX) or others, to the fullest extent permitted by Virginia law.
3. Compliance With Rules
Participant agrees to comply with all rules, policies, instructions, and safety requirements established by Beltway Lacrosse (BLX), facility operators, officials, or staff, and understands that failure to do so may result in removal from participation without refund.
4. Release and Waiver of Liability
To the fullest extent permitted by the laws of the Commonwealth of Virginia, Participant, on behalf of themselves and their heirs, executors, administrators, personal representatives, successors, and assigns, hereby releases, waives, discharges, and holds harmless:
Beltway Lacrosse (BLX) and its owners, members, officers, directors, employees, contractors, agents, volunteers, officials, other participants, sponsors, affiliates, advisors, and the owners and lessors of any facilities used for the Program (collectively, the “Releasees”)
from any and all claims, demands, causes of action, damages, losses, costs, or expenses arising out of or related to participation in the Program, including claims arising from the ordinary negligence of the Releasees, to the fullest extent permitted by Virginia law.
This release does not apply to acts of gross negligence or willful misconduct to the extent such limitation is prohibited by Virginia law.
5. Indemnification
Participant agrees to indemnify, defend, and hold harmless the Releasees from and against any and all claims, demands, damages, losses, liabilities, costs, and attorneys’ fees arising out of or related to Participant’s acts or omissions during participation in the Program, to the extent permitted by Virginia law.
6. Communicable Disease Assumption of Risk
Participant understands that participation in the Program may expose them to communicable diseases, including but not limited to COVID-19, influenza, or other viruses or illnesses. Participant voluntarily assumes all risks related to such exposure and agrees that Beltway Lacrosse (BLX) shall not be liable for any illness, injury, or complication resulting therefrom.
7. Image and Likeness Release
Participant grants Beltway Lacrosse (BLX) the irrevocable, royalty-free right to use Participant’s name, image, likeness, voice, and appearance in photographs, video, or other media for promotional, marketing, and commercial purposes without compensation.
8. Governing Law and Venue
This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia. Any legal action arising from this Agreement shall be brought exclusively in a court of competent jurisdiction located in Virginia.
9. Severability
If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall continue in full force and effect.
10. Electronic Signature
By selecting “I Accept” or signing electronically, Participant acknowledges that they have read, understood, and voluntarily agreed to this Agreement. Participant agrees that their electronic signature is the legal equivalent of a handwritten signature and that no third-party verification is required for enforceability.
PARTICIPANT ACKNOWLEDGMENT
I UNDERSTAND THAT BY SIGNING THIS AGREEMENT I AM GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE BELTWAY LACROSSE (BLX) FOR ORDINARY NEGLIGENCE. I SIGN THIS AGREEMENT VOLUNTARILY AND WITH FULL KNOWLEDGE OF ITS LEGAL EFFECT.
