Calgary Royals Athletic Association Waiver
I/We, being the Parent(s) or Guardian(s) of the above player; do hereby on behalf of the said player, release and forever discharge the Calgary Royals Athletic Association(“CRAA”) (hereafter referred to as “The Association”), their Coaches, Assistants, Directors, Servants, Employees and/or any other agents so appointed by the Volunteer Board of Directors from any and all claims whatsoever, either past, present or future, whether involving negligence or not, arising out of or in any activities of the CRAA, their coaches, assistants, directors, servants, employees and voluntary workers with respect to any claim demand, action, or cause of action arising from the activities of the CRAA.
I understand and agree to the mandatory requirements of the Respect In Sport program as a condition of registration of my child.
I understand that my registration for the current season will be denied until all outstanding fees from previous seasons are paid in full, including any assessed late fees.
I acknowledge that I meet the 'Residential Qualifications' as of September 1st for the Association's boundaries or have an 'approved release' to play. I understand that I may need to provide proof of residency at any time.
I have read and understood the Hockey Calgary and Associations, and Hockey Alberta, eligibility requirements as a condition of registration of my child.
CSEC Waiver: From time to time the Calgary Sports and Entertainment Corporation would like to communicate information directly to Hockey Calgary Members on things such as Minor Hockey fundraising, special experiences, prize draws, and free ticket offers.
Consent to Post or Publish Player Information: Hockey Calgary would like to share information and communicate with parents by highlighting Associations, Players and Player activities in a variety of public forums for non-profit purposes. The following are examples only and are not meant to be an all- inclusive list of how Player personal information and activities may be shown: displays during HC sponsored events; development sessions; other HC related activities, HC sites; HC publications; or posting or publishing on Hockey Calgary’s web site. Players’ photographs and activities that are showcased may be identified using the Player's first name, last name or initial and Association only. In order to comply with Alberta's Personal Information Protection Act (PIPA) and the Copyright Act, Hockey Calgary is requesting consent from parents to post or publish Player information and activities on various public forums. Please note the following information:
- Once photographs, Player names and other identifying information or Player activities are released in any public forum, Hockey Calgary cannot control or prevent the further distribution or use of the material by those who access the information.
- Your consent will be valid for the duration of the Hockey Season. You may withdraw your consent prior to posting or publication by notifying the Executive Director in writing. You may at any time instruct us to remove the photographs or activities from Hockey Calgary sites such as Hockey Calgary displays or from Hockey Calgary’s web site by notifying the Executive Director in writing.
Consent to Receive Communication from Hockey Calgary: Hockey Calgary would like to share information and communicate directly with parents from time to time. In order to comply with Alberta's Personal Information Protection Act (PIPA) and the Copyright Act, Hockey Calgary is requesting consent from parents to receive direct communication from Hockey Calgary. Please note the following information:
- Your consent will be valid for the duration of the Hockey Season. You may withdraw your consent prior to posting or publication by notifying the Executive Director in writing.

I, being the parent/legal guardian of the Player named in this registration, have read and understand the information provided on this agreement.

By checking the box beneath; you agree to the terms and conditions of this Agreement, the Association By-laws, and any documents included by reference. You further agree that this User Agreement forms a legally binding contract between you and the Association, and that this Agreement constitutes 'a writing signed by you' under any applicable law or regulation. Any rights not expressly granted herein are reserved by the Association. This Agreement is subject to change by the Association without prior notice (unless prior notice is required by law), by posting of the revised Agreement on the Association website.
