CALTAF

1. In consideration for accepting me as a Caltaf Athletic Association ("Caltaf") member and allowing me to
participate in Caltaf Activity, as defined below, I acknowledge that:

1.1 As a member of Caltaf I may participate in a number of activities throughout the year including but not
limited to track and filed meets, cross country meets, road races, practices, training sessions,
ceremonies, team building activities, training camps, other like activities, and travel to these activities
(collectively "Caltaf Activity").

1.2 I am familiar with and accept that there is always the risk of serious injury and death resulting from
participation in Caltaf Activity;

1.3 I have satisfied myself and believe, that I am physically, emotionally, and mentally able to participate
in Caltaf Actvity and that any equipment in my possession that I may use in Caltaf Actvity, if
applicable, is appropriate for use in Caltaf Actvity.

1.4. I understand that all applicable rules for participation in Caltaf Activity must be followed and that I
am at all times solely responsible for my personal safety; and

1.5. I will immediately remove myself from participation in Caltaf Activity, and notify the nearest coach,
official, volunteer, or medical professional if at any time I sense or observe any unusual hazard or
unsafe condition or if I feel that I have experienced any deterioration in my physical, emotional or
mental fitness, or that of my equipment, or exceeded my comfort level, for continued participation in
any Caltaf Activity.

2. I assume all risks associated with my participation in any Caltaf Activity even if arising from the negligence
on the part of Caltaf or any of its directors, officers, employees, coaches, leaders, contractors, agents,
representatives, advertisers, volunteers, and members (collectively the "Releasees").

3. I release and waive all liability and claims that I have or may in the future have against the Releasees from
any and all liability for any loss, damage, injury, or expense that I may suffer as a result of my participation
in or presence at any Caltaf Activity due to any cause whatsoever, including negligence.

4. I agree not to sue the Releasees for any loss, injury, costs or damages of any form or type, however caused
or arising, and whether directly or indirectly, from my participation in any Caltaf Activity; and

5. I agree to indemnify and save harmless the Releasees from and against all manner of litigation expense,
legal fees, liability, damages, award, or cost arising out of my participation in Caltaf Activities.

6. Caltaf has my permission to render any necessary first aid emergency treatment to me. In case of a serious
accident, Caltaf will automatically call an ambulance, or take such other steps as they deem necessary in
their sole discretion to provide me with appropriate medical assistance.

I am the parent or guardian of the above Athlete. I have read and understand the above waiver, release, and indemnity agreement, and have discussed the same with the minor person signing above. I am satisfied the said minor understands the waiver, release, and indemnity and his/her obligations as set out. In consideration of the participation of my minor child/ward in Caltaf Activity, on my own behalf and on behalf of my minor child/ward, I agree to waive, release, and indemnify the Releasees according to the terms set out above.
I am aware that by signing this agreement I am waiving substantial legal rights, which my minor child/ward and I, our respective heirs, executors, administrators and next of kin may have against the Releasees.

Please download and read the Athletics Alberta Informed Consent Agreement. This agreement includes an acknowledgment of risks, a waiver of liability, and agreement to Athletics Alberta policies. To acknowledge that you have read, understood, and agree, please sign below (you do not need to fill-in the downloaded form). At this time Athletics Alberta requires that if the Athlete is under 18, both a parent/guardian and the Athlete must understand and agree to the the above regulations, policies, bylaws, and agreements.

My Athlete is under 18 years of age. By signing my name below, I acknowledge that I, as the parent/guardian of the Athlete hereby declare that I have read, understood and agree to the contents of the Athletics Alberta Informed Consent Form in its entirety. I as the parent/guardian of the Athlete hereby agree to assume full responsibility to instruct my child of the risks involved, and to inform him/her of the importance of abiding by the rules, and regulations of Athletics Alberta.

For consideration of membership with Caltaf Athletic Association ("Caltaf"), I authorize Caltaf to use my likeness and the likeness of my Athlete in photographic, video, and audio media ("Media") in any of Caltaf's publications, including but not limited to any of Caltaf's printed and digital publications and on online media services.

I irrevocably authorize Caltaf to edit, alter, copy, exhibit, publish or distribute this Media for purposes of publicizing Caltaf's programs, news updates to Caltaf members and the public, or for any other related purpose. I waive the right to inspect or approve the finished product in which my likeness or the likeness of my Athlete appears. Additionally, I waive any right to royalties or other compensation related to the use of the Media. I acknowledge that since my participation with Caltaf is voluntary, I will receive no financial compensation.

I hold harmless and release and forever discharge Caltaf from all claims, demands, and causes of action which I or my Athlete, either of our heirs, representatives, executors, administrators, or any other persons acting on either of our behalves or on behalf of either of our estates have or may have by reason of this authorization.

By signing my name below, I acknowledge that I understand and agree to the above Media Release for my under 18 athlete.
