Cloverdale Comets Diamond Sports Assoc. Inc.

Registration Fees are non-refundable (as per the current CCDSA by-laws), however there are circumstances where parents/guardians request a refund of the registration fees as there are circumstances beyond their control and are required to withdraw from club, in this instance please contact the committee to discuss the options. A Deposit of $40.00 is non-refundable and will be withheld from any refund, the current committee with discuss the remaining fees and the amount to be refunded if applicable and the treasurer will advise of the outcome.

I have read and agree to comply with the Terms and Conditions of Membership below:

1. I consent to photographs and first name being used for promotion of the Club on the website or in newsletters.
2. I consent to the Club using our email address supplied for contact purposes only.
3. I acknowledge that, as with any sport, playing Tee-ball, Softball and Baseball involves certain risks of accident and/or injury. Whilst Cloverdale Comets Diamond Sports will take all reasonable care to ensure the safety of its players, it or its Committee, cannot be held responsible for the consequences, of events that are beyond its control. Players play tee-ball, machine pitch and softball at their own risk.
4. I agree that Cloverdale Comets Diamond Sports or any committee member, official, coach, manager or umpire will not be held responsible for any injuries sustained by the above player, or any loss of any property during the game, training session or related event. 
5. I understand that the ‘player’ shirts and pants remain the property of Cloverdale Comets Diamond Sports and I will take responsibility for the care of the shirt/pants and will return it in good condition at the end of the season. If the shirt/pants are lost or damaged I will be liable for the replacement cost of the shirt/pants.
6. Cloverdale Comets Diamond Sports is a volunteer organisation which encourages player and family involvement. I understand that, for everyone to obtain maximum benefit and enjoyment from the game and the facilities provided by the Club, membership assistance is required. I am aware I will be required to assist, and agree to do so, in a variety of roles over the season which may include, but not limited to: Canteen duty, ground preparation, fundraising, scoring, coaching and training.
7. If I cannot fulfil duties, previously agreed too, I agree to organise a replacement and notify the Committee if issues present.
8. I, and the player, agree to abide by all the Game Rules and Local Club Rules during my involvement in these games, including the CCDSA Player and Spectator Codes of Conduct.
9. I agree to abide by Club Rules and By-laws of Cloverdale Comets Diamond Sports and its affiliated bodies, including TBAWA, WASA and BWA. I acknowledge that any breach of these Rules and By-laws can and will affect my child.
10. I am aware that it is the responsibility and discretion of the Committee to place players into teams where multiple teams play in the same grade/division. Players may request team replacement; however, requests cannot be guaranteed.
11. I understand that I/my child will be disqualified from playing any games with Cloverdale Comets Diamond Sports if I am financially indebted to Cloverdale Comets Diamond Sports. All fees are to be paid in full by the fourth game to ensure player insurance is activated.
12. I give consent for my child to receive medical treatment as deemed necessary by Cloverdale Comets Diamond Sports, in the event that I cannot be contacted. All medical costs incurred shall be the responsibility of the player or their parent/guardian.
13. I acknowledge that Cloverdale Comets Diamond Sports complies with the Working with Children Act 2004.
14. I consent to the release of my personal information to the Team Coach appointed by Cloverdale Comets Diamond Sports, as well as to our affiliated bodies (TBAWA, WASA and BWA) as required and to facilitate the management of the club.
