Football Canada mandates all Coaches to complete minimum NCCP certifications. Team Trainers are also recommended to complete them. Team Managers may indicate N/A.
I acknowledge that my participation as a volunteer with the Coquitlam Minor Football Association (CMFA) is contingent upon completing a Criminal Record Check (CRC). I am aware that both returning and new volunteers can obtain the CRC online using the access code 6HPBQRJGZ0. I recognize that this service is provided free of charge and is confidential for volunteers and non-profit organizations in British Columbia working with children.

By agreeing below, I confirm that I will complete the CRC prior to commencing my volunteer duties and understand the importance of maintaining a safe environment for all participants.

A criminal record check will be required every two years. If a criminal record check was completed in 2025, it will not be necessary for 2026.

I have read and understand the items in this Code of Conduct and agree to abide by the provision as they are set forth in this document. I have also read, understand and agree to the VMFL ‘Coaches Responsibilities’ as written above. I understand that non-compliance to the Code of Conduct can result in my suspension, removal or further action taken by the VMFL Executive.
