By completing this registration form, you acknowledge you are signing the above noted child up to participate in a sport at the Scott McCormick Sports Complex and that you have the authority to do so. You acknowledge your answers and the information you provided will be used to determine details regarding the child's season. Please double check your information( For example, your child's shirt size, whether or not they should be considered for AllStars, what team they played on last year, etc.) If you have any questions regarding any of the requested information, please contact us before submitting this form. You also understand that your child's shirt size, your contact phone number, and other necessary information will be shared with your child's coach. Providing incorrect information could cause your child to miss important communication attempts from the coaches and the league. We will not give refunds for those who miss practice or games because we are unable to reach you or the child to notify you of important dates.
