Waiver, Release of Liability, and Assumption of Risk Agreement

Acknowledgment of Risk

Participation in speed, agility, and athletic training involves inherent risks, including but not limited to physical injury, muscle strains, sprains, and other potential harm. While every effort is made to provide a safe environment, injuries may occur, and by participating, you voluntarily accept these risks.

Release of Liability

In consideration of the services provided by Jackson Miller and Elite Athletic Training, LLC, the following terms apply:

1. Waiver of Claims Against Elite Athletic Training, LLC:
   Elite Athletic Training, LLC, Jackson Miller, and any representatives are not liable for any claims, liabilities, demands, or causes of action arising from injuries, illnesses, or losses sustained during participation in training sessions or related activities.

2. Waiver of Claims Against West Forsyth High School:
   West Forsyth High School is not affiliated with Elite Athletic Training, LLC, and is not liable for any injuries or incidents that may occur on its premises during training sessions.

3. Medical Consent:
   In the event of an injury or emergency, consent is given for medical treatment to be administered as deemed necessary by trained personnel. Participants or their guardians are financially responsible for any medical expenses incurred.

4. Assumption of Responsibility:
   It is the responsibility of the participant or their guardian to notify Jackson Miller of any medical conditions, injuries, or limitations that may affect participation in training sessions.

Photo/Video Release (Optional)

Photos or videos taken during training sessions may be used for marketing and promotional purposes for Elite Athletic Training, LLC. No personal identifying information will be shared without additional consent.
