RESPONSIBILITIES
I am aware that the responsibilities of a Elsinore Valley Baseball Manager (Head Coach), Assistant Coach, Team Parent or Board Member include various responsibilities, on and off the field, of a youth team or the league. I realize that my conduct should be exemplary in both citizenship and sportsmanship, and I am aware of the league rules which documents the conduct expected from individuals associated with the league. As an MPB Manager (Head Coach), Assistant Coach, Team Parent or Board Member, I agree to represent my team and the league in a manner consistent with the applicable PONY rules and regulations, the By-Laws, Official Baseball Rules & Regulations, any special guidelines established by the league in which I volunteer. Should I fail to conduct my duties in a manner consistent with the applicable rules and regulations noted above, I am aware that I may be removed from my position as a Manager, Coach, Team Parent or Board Member by action of the Board of Directors.

BACKGROUND CHECK
Elsinore Valley Baseball takes the safety of the children of the league very seriously. All volunteers will be subject to a background check and are required to fill out the volunteer application, including a copy of their driver's license (cannot be expired, needs to be current). Managers, Assistant Coaches and Board Members must also complete the online Concussion Course and the online Child Abuse Mandated Reporter Training course and submit Course Completion Certifications on their Volunteer Form with us. AT NO TIME can any adult volunteer assist players in the league in any game, practice or other type of team/league event if they have not fulfilled these volunteer requirements. As a condition of volunteering, I hereby consent to the investigation and verification of all information given in this application. I give permission for the MPB/EVB organization to conduct a background check on me, which may include a review of sex offender registries, child abuse and criminal history records (Penal Code Section 11105.03). I understand that the approval of this application is conditional upon the league receiving no inappropriate information on my background.

MEDICAL
Participation with EVB & MPB includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against injury, or infectious diseases.

LEGAL
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Menifee Pony, Inc., Elsinore Valley Baseball, Pony Baseball/Softball Inc.) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM

IMPORTANT VOLUNTEER AND LIVESCAN BACKGROUND CHECK INFORMATION:
In cases of arrest history records coming back from the LiveScan DOJ background check, the league has certain convictions that will result in automatic denial of an EVB/MPB Badge and Volunteer status, and others others will be reviewed for approval or denial by our leagues legally authorized Custodian of Records. In all cases EVB/MPB reserves the right to make final decisions on Volunteer status. The league also reserves the right to remove Approved Volunteer status from any EVB or MPB Volunteer. This may be due to the background check information, it may be because a problem or history of problems between the volunteer and our league.

EVB is operated by the Menifee Pony Board of Directors, and MPB is looking for more Elsinore residents to be part of the board.
