WELCOME TO 2024 FM ATHLETICS YOUTH FOOTBALL REGISTRATION!

We are ready to celebrate our 53rd season of youth football in 2024 with YOU!

Fargo Athletics, Inc. was formed in 1972 to promote youth tackle football education and participation in the Fargo-Moorhead area. The organization is a 501(c)3 nonprofit corporation and all board members are volunteers. During the 2023 season, we registered and rostered over 1300 youth players.

The philosophy of FM Athletics is to provide an opportunity for all youth to play football, including those who might not otherwise have the chance.  It is the intent of the league to foster sportsmanship, team play, and leadership.  A limited number of scholarships are available. If you desire to pursue this option, please contact us via email (fmafootball@gmail.com).  All scholarship information is kept strictly confidential.

REGISTRATION AND FUNDRAISING FEES:

Registration and fundraising fees are combined as one amount.

TEAM PLACEMENT/ROSTERS:

Because of the complexity of rostering 1400+ players, registrants (as well as coaching candidates) cannot be guaranteed placement on the same team or with the same coaching group each year.  Additionally, we are unable to accommodate specific carpool or friend/teammate requests at this time. One component of forming teams is date of registration. (i.e. first-registered, first-rostered)

FUNDRAISING 2024

I acknowledge that I will be responsible to pay $150 (included in the registration price) up front for the price of my player's fundraising peelers. I also understand that I may re-sell the peelers (at face value of $10/each) on my own to recoup the cost. I further acknowledge that the fundraising fee is non-refundable.

Please read through each of these waivers and consents carefully.

League participants (parents, players, guardians and coaches) are expected to respect and adhere to these guidelines for the entirety of the season.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while attending sanctioned FM Athletics events and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, FM Athletics employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at any and all FM Athletics sanctioned events ("Claims"). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless FM Athletics, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the FM Athletics, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any FM Athletics sanctioned program or activity.
I am the minor’s parent or guardian named on this registration form and I understand the nature of the Waiver of Liability above and verify and consent to the minor attending events sanctioned by FM Athletics. On the minor’s behalf, I hereby release, covenant not to sue, discharge, and hold harmless FM Athletics, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. On behalf of the minor, I understand and agree that this release includes any claims based on the actions, omissions, or negligence of FM Athletics, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending in any FM Athletics sanctioned program or activity.
The undersigned parent or guardian for and in consideration of accepting this application for a member of FM Athletics, Inc., does hereby waive and release all rights and claims against said corporation, its agents, sponsors, or representatives of anyone connected with the league. Physical examinations of participants are not required and are left to parental discretion. The player participating in FM Athletics does so at his/her own risk. FM Athletics shall not be liable for any damages arising from personal injury which may occur to the player during the season and so hereby fully and forever exonerate and discharge FM Athletics, its board members, employees and agents from any and all claims, demands, damages, right of action or causes of action, present or future, whether the same be known, anticipated, or unanticipated, resulting from or arising out of the player’s participation in FM Athletics and in the use of the facilities. I accept responsibility that the athlete/child being registered through this registration process is fully able to participate in the activity without assistance of any kind and does not have any serious health concerns that would prohibit their participation or endanger themselves or their teammates.
I hereby acknowledge that verbal or physical abuse of ANY kind directed towards officials, coaches, board members or other parents and/or un-sportsman-like conduct will not be tolerated. Failure to comply with this code may result in disciplinary action and/or removal from the league. I further understand that if a participant or parent becomes a disruption to team practices or games, they may be dismissed at the board's discretion.
I hereby agree that I have accurately reported my player's weight and understand and acknowledge that the league has weight limits in place to promote and offer safer play for all participants. I confirm that my player falls within the maximum weight limit for their grade/age. I further understand that my player will be allowed ONE official weigh-in at the time of equipment issue.
I understand that the league will work to place students with teammates from the same school, but because of the complexity of rostering 1400+ players, registrants (as well as coaching candidates) cannot be guaranteed placement on the same team or with the same coaching group each year. Additionally, I acknowledge that the league is unable to accommodate specific carpool or friend/teammate requests at this time and that teams are formed on a first-registered, first-rostered basis.
I understand that my child will be issued, prior to the start of the season, a full set of FM Athletics’ property, including, but not limited to: team jersey, helmet, football pants, shoulder pads, knee pads, thigh and hip pads. All issued equipment must be returned undamaged to FM Athletics at the conclusion of the season. If the equipment is not returned, I understand that I will be liable for the replacement cost of the equipment. Participants who fail to return equipment in a timely manner and/or pay the fee are ineligible to play in upcoming seasons. This would also negate any future scholarship opportunities. All issued equipment is the property of FM Athletics and may not be used for any practice, scrimmage, game, or any other purpose which is not scheduled and sanctioned by FMA. If a player decides not to play FMA football after receiving issued equipment, the player MUST return all issued equipment to the Equipment Manager immediately. Any FMA equipment not returned upon a player deciding not to play during the current season or at the end of the current season will result in a $800 fee. Further, if a player is found to be using FMA equipment for any other organization other that FMA, the player MUST return all FMA issued equipment immediately.
I understand that baseline concussion screenings are available to my student (at parent's request, and NOT mandatory). Appointments can be made by calling Sanford Sports Medicine directly: (701) 417-6000. I further acknowledge that I have been made aware of the concussion information found at fmathletics.com (CONCUSSION tab).
Games will be played when fields are available. Games are played on Saturdays, Sundays, Mondays, Tuesdays and Thursdays.
