Waiver and Release of Liability *

Waiver and Release of Liability
1. Assumption of Risk
I understand that participation in the sport of soccer, training and physical activities involves inherent risks, including but not limited to falls, collisions, physical exertion, equipment related injuries, environment risks and other risks that may result in injury, concussions, illness, permanent disability, or death.
2. Release of Liability
To the fullest extent permitted by law, I release and discharge FFIT Soccer Academy, its owners, directors, coaches, employees, volunteers, contractors, and facility providers from any and all claims, demands, damages, actions, or causes of action arising from or related to participation in academy activities, except where caused by gross negligence or willful misconduct.

Medical/Emergency Consent *

In the event of an emergency, I authorize academy staff to obtain medical treatment for the participant if a parent or emergency contact cannot be reached promptly. I understand that all associated costs are my responsibility.

Health Declaration *

I confirm that the participant is physically fit to participate and that any relevant medical conditions, allergies, or limitations have been disclosed to the academy.

Uniform/Equipment Expectations *

I understand that the player registered under my care is required to wear athletic clothing, appropriate footwear, shin pads with socks overtop. Outdoor elements can be extreme, I understand it is my responsibility to bring appropriate clothing for participation. Water, snacks (if required), plus any medical necessities are my responsibility to provide.

Code of Conduct *

I agree to comply with all academy rules, policies, and instructions from coaches and staff. The academy reserves the right to remove participants whose conduct is unsafe or disruptive without a refund.

Program Cancellation Policy *

FFIT Soccer Academy reserves the right to cancel programs that do not have a minimum of 8 participants registered prior to Registration End Date.
FFIT Soccer Academy reserves the right to cancel a program or session if unforeseen circumstances arise due to coaches/staff emergencies.
If a program is cancelled at the discretion of FFIT Soccer Academy, participants who have registered will receive a full refund minus the online processing fee. The processing fee is based on a percent of the registration fee and is paid to the online payment provider during registration.

Session Cancellation/Reschedule Terms *

FFIT Soccer Academy will reschedule sessions that are cancelled due to staff illness or weather and environment conditions that close fields/facilities. Sessions will be rescheduled during the Block of Sessions if possible on a weekend or the week following the last session. No Refunds or Credits will be given due to rescheduling sessions for weather, environmental conditions or illness.

Photograph and Media Consent *

I grant permission for photographs and videos taken during academy activities to be used for promotional, educational, and marketing purposes without compensation.

Privacy and Data Use Consent *

By Registering with FFIT Soccer Academy, I consent to the collection, use, and storage of personal information required for program administration, communication, participant safety, emergency response, and payment processing.

Refund Policy

Registration Fee

A $20 registration/administration fee is non-refundable.
Online Payment Fee is non-refundable

Withdrawal Before Program Start

Written cancellation received 7 or more days before the first session: Full refund minus the registration fee and online payment processing fee.
Cancellation received less than 7 days before the first session: 50% refund of program fees.

Withdrawal After Program Start

No refunds once the program has started, except for documented medical reasons.

Medical Withdrawal

A prorated refund or account credit may be issued with a doctor’s note.
Refund amount will be based on the remaining unused sessions, minus the online payment processing fee.

Missed Sessions

No refunds or credits for missed classes due to vacations, scheduling conflicts, or no-shows.

Program Cancellation

If the academy cancels a program, participants will receive a full refund or account credit.

Refund Requests

All refund requests must be submitted by email.
Approved refunds will be processed within 14 business days.
