The form is for COACHES ONLY. If you intend to bring a team and have not been issued a team code, please message us on Facebook or call/text us at 606-369-6144. Coaches and players will need this code to complete registration. Space is limited and will be issued on a first come, first serve basis. 

COACHES, DO NOT REGISTER YOUR PLAYERS. They must be registered by a parent or guardian, provide an email address, and acknowledge the participation waiver, to be eligible. This is also how their account is created for the free mobile app as well. Please encourage your parents and players to use this app as it is how the league will send out important messages such as schedule changes, weather cancellations and delays. It's also a great tool for you, with roster information, parent contacts, team chat, schedules, scores etc..

READ BEFORE AGREEING -- RELEASE OF LIABILITY FOR PARTICIPANTS  --  CODE OF CONDUCT AGREEMENT -- READ BEFORE AGREEING


IN CONSIDERATION OF myself, my child/ward, being allowed to participate in any way in the 606 Sports LLC, East Kentucky Baseball, Floyd County Baseball, and affiliates related events and activities, undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child/ward's participation; and,
3. I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward's readiness for participation and/or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately; and,
4. I for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS 606 Sports LLC, East Kentucky Baseball, Floyd County Baseball, and affiliates, their directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ('Releasees'), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
5. I, for myself, my spouse, my child's/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child's/ward involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

UNDERSTANDING OF RISK
I understand the seriousness of the risks involved in participating in this program, my personal responsibilities for adhering to rules and regulation, and accept them as a participant.

POLICIES and CODE OF CONDUCT 

  This League is designed to provide a structured, supervised, instructional and competitive baseball program for any and all youth age 13 to 19 within the designated boundaries of of eastern Kentucky, which fosters an environment that emphasizes sportsmanship, fair play, physical and mental well-being, teamwork, baseball skills development, community involvement, volunteerism, the development of good citizens and positive role models for the community.

The following codes of conduct will be strictly enforced throughout every event.

SPECTATORS CODE OF CONDUCT
In order to ensure that the principles of sportsmanship, fair play, skill development, and mutual respect among players, managers, coaches, umpires and spectators are the primary considerations governing competition in the East Kentucky Babe Ruth League, the following Code of Conduct has been established and adopted.

Each spectator will ensure that his/her conduct at all games and practices are that of a mature adult with the best interests of every ball player and the East KentuckyBabe Ruth League, continually expressed. To satisfactorily meet these responsibilities, every spectator is expected to:

Respect the rules of the game.
Respect the umpires, managers, coaches and players and refrain from questioning their decisions or from addressing them in a loud, disrespectful or abusive manner
Cheer for their team in a positive, supportive manner, refraining at all times from making hostile, negative, or abusive remarks about the opposing team.
Adhere to all practice/game facility rules and regulations.
Demonstrate appropriate gestures of sportsmanship at the conclusion of a game, win or lose.
Realize, accept, and practice the principal that a team reputation is built not only on its playing ability, but also on the sportsmanship, courtesy, and citizenship of its fans.
Refrain from the use of alcoholic beverages, tobacco products or other illegal substances at games, practices and all other EKBR team events.

RESPONSIBILITY FOR CONDUCT
 Each team is responsible to see that their managers, coaches, players and spectators abide by the respective CODE of CONDUCT. All managers, coaches, players and parents or guardians of players will be provided copies of the appropriate CODE of CONDUCT Acknowledgment forms prior to the start of the season.

CODE OF CONDUCT REQUIREMENTS
CODE of CONDUCT for spectators and players should be thoroughly discussed at a pre-season joint meeting between managers, coaches, players and parent/guardians of players. All sponsors, coaches, parents/guardians and players are required to sign the CODE of CONDUCT Acknowledgement form indicating they understand and accept their responsibilities as stated in the code. This form, with the required signatures, is to be filed with EKBR.

IMPROPER CONDUCT ADJUDICATED BY EKBR
Conduct of sponsors, managers, coaches, players or spectators detrimental to EKBR or contrary to the EKBR CODE of CONDUCT may result in suspension from further participation for whatever period of time deemed reasonable and appropriate by 606 Sports management.

IMPROPER CONDUCT PENALTIES
Improper conduct on the part of managers, coaches, players or spectators may result in expulsion from the game/practice facility, or in extreme cases, forfeiture of the game. EKBR reserves the authority to discipline managers, coaches, players or spectators during the course of the season for conduct deemed detrimental to the good order of the League. Such actions may include warning, suspension or termination. In the event of termination, no refunds of registration fees will be provided.

OFFENSIVE ACTIONS 
DEFINITIONS OF OFFENSIVE ACTIONS
No manager, coach, player, parent/guardian or spectator is to:
refuse to abide by an umpire's decision; or
be guilty of objectionable demonstration by throwing equipment or any other forceful action: or
be guilty of heaping verbal abuse upon an umpire for any real or imaginary wrong decision or judgment; or
be guilty of an abusive verbal attack upon a player, an opposing manager, coach, umpire or spectator; or
at any time, lay hands upon, push, shove or strike an umpire, manager, coach, player or spectator; or
be guilty of a physical attack as an aggressor upon any player, opposing manager, coach, umpire, spectator or league official.
engage in any activity that is deemed detrimental to EKBR or the good order of the league.

PENALTIES FOR OFFENSIVE ACTIONS/EJECTION FROM GAMES
All ejections will be considered CODE of CONDUCT violations, and the manager, coach or player is automatically suspended for at least ONE (1) game. EKBR management will define any further appropriate penalties and must approve permission for any suspended player, manager or coach to participate in games subsequent to the incident for which they were ejected. All rulings will be expedient, provided that both team managers and the umpire association reports the facts relative to the ejection promptly. A player or manager ejected from a game should be notified at the time as to the cause of the ejection; both managers are still required to notify the League Commissioner when an ejection occurs. A written report by the ejecting umpire explaining the reason(s) for the ejection should be made in all situations involving ejection. Any manager, coach, player, team representative, or parent/guardian striking or shoving an umpire will be suspended from attending any EKBR activity for the remainder of the season and would no longer be eligible for participation in any league activities in the future..

SUBMISSION OF APPEALS
A written appeal may be made to East Kentucky Babe Ruth within TWENTY-FOUR (24) hours of the penalty decision. The decision of EKBR management will be final.

BY AGREEING TO THESE TERMS, YOU AGREE TO EACH SECTION OF THE CONTRACT THAT APPLIES AND WILL COMPLY WITH THIS CONTRACT, THE TEAM RULES AND ALL EKBR GUIDELINES. YOU ALSO AGREE ABOUT THE IMPORTANCE OF SPORTSMANSHIP AND DEMONSTRATING POSITIVE BEHAVIOUR AT ALL BASEBALL EVENTS. FAILURE TO AGREE TO THE TERMS WILL RESULT IN THE PLAYER NOT BEING ABLE TO PARTICIPATE IN ANY TEAM ACTIVITY.
