Purpose: This form allows the Forest Lake Youth Lacrosse Board to follow a standardized process for the review and selection of applicants to coaching, team manager, or other coach support staff positions. Having prior coaching or volunteer experience at any level is not a requirement for application with Forest Lake Youth Lacrosse.

Privacy Policy: Forest Lake Youth Lacrosse will collect information from all coaching applicants for the purposes of determining eligibility and suitability and to conduct background investigations necessary to insure the safety of our participants. We will not collect any information that is not reasonably necessary to participate in this program. Except for the information necessary to conduct background investigations, we will not disclose applicant information to any third party.

Instructions: Please fill out ALL information requested. No person shall be considered for any position until a completed application is submitted and approved by the Forest Lake Youth Lacrosse Board. Upon acceptance of any coaching position, you understand that failure to comply with all rules can result in termination, without the ability to apply to future open coaching positions. All applications are subject to review and approval by the Forest Lake Youth Lacrosse Board, and in some cases, it’s executive board, Forest Lake Area Athletic Association (FLAAA).

If you are selected to a coaching position, you must agree to the following:

I understand I will need to submit a background check through FLAAA. This background check will be paid by Forest Lake Boy’s Youth Lacrosse association and will be valid for 2 years.

I understand I must be a current member, or working towards completion, of US Lacrosse registered in a coaching category. This fee can be reimbursed by Forest Lake Youth Lacrosse with the proper documentation. Get started with becoming a member here: https://www.usalacrosse.com/membership.

I agree to take the SafeSport training, or provide proof of a certification already in good-standing. *After registration with US Lacrosse, you will be redirected to this training. Important: if you have done this for another sport, you do not have to retake this class and can link your completion to the USA Lacrosse account by following the instructions on the page.

I understand that I can request to attend coaching clinics at the expense of Forest Lake Youth Lacrosse association. Authorization is required by the committee prior to registering for these clinics.

I understand and accept the importance of attending all scheduled practices and games and will be present for them. If I am unable to attend the practice/game I will give advanced notice so that a replacement can be arranged.

I understand that it is my responsibility to train and mold players under my guidance and direction. While I acknowledge that I have an obligation to my players to do my best to win games, I also have an obligation to promote the growth of each player’s skills and maturity. To this end, I will attempt to give each player who has shown drive and desire to learn the game by regularly attending practice and showing a good attitude, playing time in each game.

I understand and accept that it is my responsibility: (1) to learn and abide by the rules of the lacrosse set out in the governing rule book (2) to ask questions of officials and league representatives for clarification when necessary and at the appropriate time and (3) to make sure all players under my guidance and direction use all mandatory equipment in the proper manner and to notify individual players if their equipment is illegal or becomes illegal to use during the course of play.

I understand and accept that the Forest Lake Youth Lacrosse association requires that an atmosphere of good sportsmanship be maintained at all of its athletic competitions and that unsportsmanlike conduct will not be tolerated. No Coach shall:

Enter into an argument with an official as to any decision, or in any way attempt to influence the decision of an official.
Use threatening, profane or obscene language or gestures at any time.
Bait, taunt, call undue attention to oneself or any other act considered unsportsmanlike by the officials.

I understand that if I experience any situation with a parent, player, referee, or members of any opposing team that is construed as inappropriate, I will report this to the respective Boys and/or Girls Directors immediately; if the situation becomes hostile or abusive in any way, I agree to call law enforcement and comply will law enforcement requirements or investigations.

I understand and agree that I will not participate in any illegal activity and if this should occur, I will immediately be dismissed from my coaching position.

I understand that if I am considered a paid coach, I am required to fill out the coaches document issued by FLAAA. This document states it is FLAAA’s responsibility to issue an IRS form 1099 if these earnings are at or over $600, as required by Federal law. You will be provided a fee schedule and instructions on how to report your time.

I understand that I will be provided the payment schedule, which is based on position and years of experience, and I must accurately complete all required documentation throughout the season to ensure proper payment can be issued.
