Golden Minor Soccer

1. It is Golden Minor Soccer Association's policy to notify the parent when a player is ill or requires medical attention. If we are unable to contact the parent and the player requires immediate medical help, parental consent is necessary for the association to take appropriate action on behalf of the player. I authorize the assocation to call a physician, take my child to the nearest emergency centre or summon an ambulance for emergency medical aid should the person(s) in attendance feel such services are required and I can not be contacted by telephone. If such emergency should arise, I shall be notified as soon as possible. I agree that any costs incurred for such services shall be the sole responsibility of myself.

2. I agree to pay the soccer fees outlined in this registration form by paying by credit card now. I acknowledge that if the soccer fees are not paid in full by April 1st, 2025, my child will not be able to participate in the 2025 soccer season.

3. PARENT CODE OF CONDUCT

Please read this carefully as we expect all our volunteers, parents and athletes be treated with respect and kindness. No exceptions.

1.  Remember that your athletes are involved in organized soccer for their enjoyment, not yours.
2. Encourage your athlete to always play by the rules.
3. Teach your athlete that an honest effort is as important as victory so that the result of each game is accepted without undue disappointment.
4. Turn defeat to victory by helping your son/daughter to work toward skill improvement and good sportsmanship. Never ridicule or yell at your athlete for making a mistake.
5. Provide your athlete with plenty of encouragement because that is the best way they learn.
6. Show respect and appreciation for the volunteer coaches who give their time to provide sport activities for your athlete. Do not publicly question the coaches or manager’s judgment and never question their honesty.
7. Never openly criticize the performance of a referee or an assistant referee.
8. Support all efforts to remove verbal and physical abuse from children’s sporting activities.
9. Please let your athlete speak on their behalf.

If, for any reason, your athlete can't participate in the season PRIOR to season start, we will consider a full refund. Please keep in mind that this is all a lot of work so make sure your athlete wants to be registered prior to doing so.
