Please review all of the following disclaimers prior to submitting your registration. Note that, by completing this section, you will waive certain legal rights, including the right to seek legal address (i.e. the right to sue us) following any accident, illness, disability or injury, both against the Halifax Players' League for Affordable Yearly Sports Association (PLAYS), incorporated under NS Registry of Joint Stocks # 3281353, as well as the Association's volunteers, employees and executives. By ticking the checkbox below and submitting your registration, you will have indicated that you have FULLY READ, UNDERSTOOD AND AGREED TO OUR WAIVERS, listed as follows:
- I verify that I am 19 years of age or older at the time of completing this registration.
- I understand that Halifax PLAYS (referred to as 'it'), or its volunteers, employees, executives & facilities rented by it, are not responsible, financially or otherwise, for injuries, accidents, disabilities, illnesses or death incurred during athletic play organized by it. I also understand that I will be ineligible for a refund of registration costs in case of such occurrence.
- I understand that Halifax PLAYS, or its volunteers, staff and executives, are not responsible, financially or otherwise, for loss or damage to personal possessions, such as equipment, electronics or eyeglasses, incurred during play organized by it.
- I understand that having any Halifax PLAYS program is contingent on many factors, and that there is a significant possibility that the program I am signing up for will not happen in the end, or that details of the league will be different than what was advertised. Even if the program is feasible to run, I understand that it may not necessarily be able to accommodate me or my preferences, including preferences for teammates I would want to play with.
- I understand that my continued membership as a player or volunteer in any Halifax PLAYS program is at the discretion of its organizers and the Association’s Board of Directors. As well, I understand and am aware of the Disciplinary Policy of Halifax PLAYS outlined on its website at www.Halifaxplays.com/board, and will respect the authority of League Representatives to enforce discipline against me, at their discretion, as outlined in the Policy. This includes, but is not limited to, revoking my membership of Halifax PLAYS or its Programs without refund (i.e. you might get kicked out of the league) for failure to abide by league rules and regulations, such as disruptive behavior, unsportsmanlike conduct, discriminatory speech, failure to promptly pay fees, or other legitimate reasons as outlined in the Policy and/ or determined by the Halifax PLAYS Board of Directors or league organizers.
- I understand that participation in any Halifax PLAYS program includes possible exposure to and illness from infectious diseases, including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce risk, the possibility of serious illness and death does exist, and I knowingly and freely assume all such risks- known and unknown- involved in participating in any Halifax PLAYS program, even if arising from the negligence of Halifax PLAYS, its volunteers, employees, executives, participants or others.
- I also agree to comply with the stated and customary terms and conditions for participation, as regards protection against infectious diseases, and in particular the return-to-play guidance documents developed by Halifax PLAYS.
- I understand that it is my responsibility to observe for any unusual or significant hazards during my presence, and to remove myself from participation and bring any such hazard to the attention of a league official or representative immediately.
- I understand that information in this registration will be shared with team captains, volunteers, employees and executives in the Halifax PLAYS organization, and accept that Halifax PLAYS is unable to monitor every use of this information by all the above individuals.
- I understand that Halifax PLAYS, including its executives, employees and volunteers, may take images or videos of me during the course of any play or events organized by it. I understand that these may be used at the discretion of league organizers for the purposes of promoting Halifax PLAYS, such as by posting these images on the league website, putting them on posters, including them in media stories, or for other purposes deemed appropriate by the Halifax PLAYS organization. I further understand that if I do not wish for my image to be displayed, I must confirm this in writing to Halifax PLAYS.
- I understand that Halifax PLAYS may add my e-mail to its mailing list, so as to send me notifications about its programs, including but not limited to the programs I am signing up for. I also understand that I may unsubscribe myself from this mailing list at any time by visiting halifaxplays.com/mailing-list/unsubscribe.
- In completing this Release Agreement, I elect to use the facilities and equipment provided or approved by Halifax PLAYS at my own risk, and do thereby release Halifax PLAYS, its volunteers, employees and executives together with their heirs, assigns, officers, representatives, agents, employees and members, sponsoring organization and owners of properties or facilities, from all liability from injury, illness, accident or death to person, property, and/ or reputation that I may receive, and from all claims of said injuries growing out of, or resulting from, my participation in a program at Halifax PLAYS.
- I agree to save harmless and keep indemnified Halifax PLAYS, its rented facilities, its volunteers, employees and executives, together with their heirs, assigns, officers, representatives, agents, employees and members, sponsoring organization and owners of properties or facilities, from all liability from with respect to death, illness, injury, loss or damage to my person or property and/or reputation that I may receive and from all claims of said injuries growing out of, or resulting from my participation in a program at Halifax PLAYS.
- I confirm that I have READ AND UNDERSTOOD the release agreements above prior to indicating my approval for them with the checkbox below. As well, I am aware that by agreeing to this release agreement, I am waiving certain legal rights which I, or my heirs, next of kin, executors, administrators, assigns, and representatives, may have against the Halifax PLAYS Association or its volunteers, executives, rented facilities, employees, board members or organizers.
