Anyone playing that is not registered will be asked to leave and not be allowed to play in any HHMVL Tournaments. By registering your team you are registering yourself to play in the 2024/2025 Volleyball season. This is an insurance matter not a league matter. This is your responsibilty to ensure all your players have regsitered. Any questions please email: Registration help

WAIVER, RELEASE, COVENANT NOT TO SUE & INDEMNITY AGREEMENT

Halton Hills Mixed Volleyball League (HHMVL)

IMPORTANT: All players must be a minimum of 19 years of age by the first match. This registration and waiver must be submitted with registration. If an individual fails to complete a waiver they will not be permitted to play and matches may be forfeited. Only registered players are permitted to play in HHMVL events and no spectators under the age of 19 years will be permitted in any facility used by the league.

WAIVER, RELEASE, COVENANT NOT TO SUE & INDEMNITY AGREEMENT

I understand and accept that there are risks involved in participating in any recreational activity. I am aware of those risks and I am voluntarily participating in this activity with knowledge of the risks involved. I agree to accept any and all such risks of injury, death and/or property damage. I agree to the terms of this Waiver, Release, Covenant Not To Sue & Indemnity Agreement as set forth herein.

I hereby release, absolve, indemnify and agree to hold harmless the Halton Hills Mixed Volleyball League (HHMVL), its executive, members, volunteers, agencies and any other representatives of the HHMVL from any and all claims, damages, demands, losses, expenses and ability arising from any accident, injury or illness which is caused by or arises from my participation hereon during any HHMVL activity or in any location where the activity is being held.

In case of injury or illness, I give my permission for emergency transportation and the administration of first-aid, medical and/or dental treatment if necessary. I accept responsibility for the payment of any emergency transportation, treatment expenses and any related or subsequent medical and/or dental bills.

In addition, I understand that my conduct, if deemed inconsistent with the rules of good sportsmanship and fair play or the HHMVL Constitution, may result in my expulsion from all HHMVL programs. Any photographs taken while participating in any program, recreational activity, or event are the property of the HHMVL and may be used at their discretion. This waiver will cover all participation in any said HHMVL event for a period of 1 year from the day it is dated and accepted or signed.

Prior to accepting the terms of this Waiver, Release, Covenant Not To Sue & Indemnity Agreement I have fully read its content and understand it’s meaning and legal impact thereof. I voluntarily, of my own free will and without distress or coercion sign this waiver, release, covenant not to sue & indemnity agreement. I hereby promise to read and abide by the HHMVL Constitution found at www.hhmvl.com.
