Hockey Calgary

Hockey Calgary would like to share information and communicate with parents by highlighting Associations, Players and Player activities in a variety of public forums for non-profit purposes. The following are examples only and are not meant to be an all-inclusive list of how Player personal information and activities may be shown: displays during HC sponsored events; development sessions; other HC related activities, HC sites; HC publications; or posting or publishing on Hockey Calgary’s web site. Players’ photographs and activities that are showcased may be identified using the Player's first name, last name or initial and Association only. In order to comply with Alberta's Personal Information Protection Act (PIPA) and the Copyright Act, Hockey Calgary is requesting consent from parents to post or publish Player information and activities on various public forums.

Please note the following information:
• Once photographs, Player names and other identifying information or Player activities are released in any public forum, Hockey Calgary cannot control or prevent the further distribution or use of the material by those who access the information.
• Parents are under no obligation to consent; it is their voluntary decision to do so. If you do not agree to this document, this indicates a refusal to consent.
• Your consent will be valid for the duration of the Hockey Season. You may withdraw your consent prior to posting or publication by notifying the Executive Director in writing. You may at any time instruct us to remove the photographs or activities from Hockey Calgary sites such as Hockey Calgary displays or from Hockey Calgary’s web site by notifying the Executive Director in writing.

Consent for Release
I, being the parent/legal guardian of the Player named in this registration, have read and understand the information provided in this agreement. I voluntarily give Hockey Calgary consent to post or publish my or my player's photographs, first name and last name or initial, Association and activities in various public forums as described above for non-profit purposes. I understand my consent is valid for the current Hockey Season unless I revoke my consent prior to posting or publication by notifying the Executive Director in writing.

Hockey Calgary would like to share information and communicate directly with parents from time to time. In order to comply with Alberta's Personal Information Protection Act (PIPA) and the Copyright Act, Hockey Calgary is requesting consent from parents to receive direct communication from Hockey Calgary.

Please note the following information:
• Parents are under no obligation to consent; it is their voluntary decision to do so. If you do not agree to this document, this indicates a refusal to consent.
• Your consent will be valid for the duration of the Hockey Season. You may withdraw your consent prior to posting or publication by notifying the Executive Director in writing.

Consent for Communication
I, being the parent/legal guardian of the Player named in this registration, have read and understand the information provided in this agreement. I voluntarily allow Hockey Calgary to provide communication from time to time to me directly. I understand my consent is valid for the current Hockey Season unless I revoke my consent prior to posting or publication by notifying the Executive Director in writing.

From time to time the Calgary Sports and Entertainment Corporation would like to communicate information directly to Hockey Calgary Members on things such as Minor Hockey fundraising, special experiences, prize draws, and free ticket offers.

I do hereby give consent and I certify that I am the Parent or Legal Guardian of the registered child(ren).

Every reasonable precaution has been taken to ensure the safety of the event and the participants.

I/We the Parent(s) / Guardian(s) of the person(s) being registered, hereby give approval of the person(s) to being registered in the Jr Flames Program, as noted in the registration.

I/We understand and assume all risks and hazards of the mentioned person(s) in these activities as well as all risks and hazards incidental to the activities and transportation to and from the activities.

I understand that my or my Child’s participation in the Jr Flames Program will increase the risk of exposure to or contracting COVID-19, by the very nature of the activity, circumstances and environment, and we are taking part in this program/activity at our own risk.

I hereby undertake and agree that we will always act prudently and responsibly – all in order to avoid risk and ensure the health, well-being and safety of others, myself and my Child.

I/We do further hereby release, indemnify, hold blameless and waive any and all claims directly or indirectly due to participation in the Jr Flames Program; that I have or may have in the future against the Jr Flames Program, Hockey Canada, Hockey Alberta, Hockey Calgary, Calgary Sports and Entertainment Corporation and its members, officers, employees, agents, volunteers, independent contractors and facility personnel.

By signing this agreement, I am confirming that the participant I am registrering is not currently enrolled in any regular stream league - Minor or House League - sanctioned by Hockey Calgary.
