By registering, I agree to the following terms and conditions while my child is participating in activities sponsored by Jasper Minor Sports:

1. To take proper care of the equipment provided by JMS and to return it at the end of the season, or be responsible for the replacement cost.

2. To see that the property of others is not mistreated.

3. To be honest & courteous & refrain from profane language and to treat all player, coaches, assistants, & referees with dignity and respect.

4. I authorize JMS as represented by a director, coach, referee, or manager to obtain medical aid for my child if they deem it necessary.

5. I have listed below any physical impairment, illness, or allergy of which I am aware that might affect my child's participation in Hockey and related activities.

6. JMS photos may be posted on the internet.

7. I assume all liability for any accidents that may occur as a result of my child's participation in this activity.

8. PLAYERS FROM OTHER MINOR SPORTS INSTITUTIONS WILL NOT BE ALLOWED TO PARTAKE IN JMS PRACTICES UNLESS OTHERWISE DISCUSSED WITH THE BOARD PRIOR TO PRACTICE TIME, IN WHICH AN INSURANCE WAIVER WILL HAVE TO BE SIGNED BY THE PLAYER.
