I agree to the following:

THIS IS A LEGAL AGREEMENT. UNDER THIS AGREEMENT, YOU ARE ASKED TO WAIVE (GIVE UP) CERTAIN RIGHTS YOU MAY HAVE. IF YOU SIGN THIS AGREEMENT YOU GIVE UP ALL OF THOSE RIGHTS. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT AND UNDERSTAND IT.

SECTION 1

I agree to the following:

GENERAL RELEASE & WAIVER For good and valuable consideration, the sufficiency of which I, THE ABOVE NAMED PARENT or LEGAL GUARDIAN, hereby acknowledge, I, personally and on behalf of and as the parent and/or legal guardian of THE ABOVE NAMED PARTICIPATING PLAYER, and anyone acting on my behalf, including but not limited to attorneys, representatives, agents, heirs, executors, administrators, assigns, insurers, predecessors, successors or any other person or entity asserting claims through me, agree to release, indemnify, defend and hold harmless: Jr Pacers Youth Football and Cheer and all of their respective directors, officers, shareholders, subsidiaries, partners, agents, employees, successors, parents, beneficiaries, heirs, executors, administrators, assigns and affiliates thereof (collectively, "Releasees"), from and against any and all claims, suits, losses, damages, expenses, costs, and liabilities (including reasonable attorneys' and consultants' fees and expenses) which hereinafter may accrue or arise against the Releasees and which in any way arise out of or are in anyway related to (a) my child's participation in the Jr Pacers Youth Football and Cheer Football Seasons and/or (b) the use of my child's name, photograph, quotation, and likeness in any advertising or promotions which relate to the Seasons or other Jr Pacers Youth Football and Cheer promotions.

SECTION 2

I Agree to the following:

I, PERSONALLY AND ON BEHALF OF MY CHILD, UNDERSTAND THE DANGER OF PERSONAL INJURY OR DEATH THAT MAY RESULT FROM PLAYING FOOTBALL AND PARTICIPATING IN THE SEASONS INCLUDING BUT NOT LIMITED TO FALLING, BEING STRUCK BY THE FOOTBALL OR ANOTHER INDIVIDUAL.

SECTION 3

I agree to the following:

I, PERSONALLY AND ON BEHALF OF MY CHILD, ASSUME ANY AND ALL RISK THAT IS IN ANY WAY ASSOCIATED WITH, RELATED TO OR OCCURS AS A RESULT OF MY CHILD'S PARTICIPATION IN THE SEASONS.

SECTION 4

I agree to the following:

I, PERSONALLY AND ON BEHALF OF MY CHILD, HEREBY GIVE RELEASEES, OR ANY ONE OF THEM, PERMISSION TO USE MY AND/OR MY CHILD'S NAME, PHOTOGRAPH, QUOTATIONS AND LIKENESS IN ANY ADVERTISEMENTS OR PROMOTIONS PERFORMED IN CONNECTION WITH THE SEASONS AND AGREE THAT NEITHER I NOR MY CHILD SHALL BE ENTITLED TO ANY COMPENSATION FOR SUCH USE.

SECTION 5

I agree to the following:

I, HEREBY CERTIFY THAT I AM 18 YEARS OF AGE OR OLDER AND REPRESENT THAT EITHER I AM THE PARENT OR LEGAL GUARDIAN OF THE PARTICIPANT NAMED ABOVE.

SECTION 6

I agree to the following:

This section is for Electronic Registrants of the Season only. By completing and submitting the electronic/online registration (including a mandatory response that you understand and accept this waiver) I certify that I have 1) read the "Jr Pacers Youth Football and Cheer” waiver, 2) that I am the custodial parent(s) and/or legal guardian(s) of the person designated as Participant above, and 3) that I am over the age of eighteen (18) years of age. This action will constitute an electronic signature in lieu of an actual signature and evidences the parties’ mutual intent to enter into a valid and binding contract that will satisfy all applicable legal requirements. This Agreement shall constitute an “original” when printed from electronic files or records established and maintained in the normal course of business.

SECTION 7

I agree to the following:

Parent / Family / Guests of Jr Pacers Youth Football and Cheer.

will conduct themselves in an exemplary manner at games and practices.
will clap or cheer, always in a positive manner only, never negative or unsportsman like.
Zero comments to Officials, coaches, or opposing teams. (zero tolerance)
No drugs, alcohol, or gambling is permitted at any Jr Pacers event, game or practice.
No artificial noise makers are permitted at Jr Pacers home or away games.
Violation of any of these league policies will result in expulsion from Jr Pacers events.
