THOCKEY PROGRAM WAIVER & CODE OF CONDUCT

Assumption of Risk: I understand that ice hockey is a vigorous physical sport involving inherent risks, including but not limited to serious physical injury. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my child’s participation.

Medical Authorization: In the event of an injury or illness during a JTHockey session, I authorize the staff to secure medical treatment for my child if I cannot be reached.

Media Release: I grant JTHockey permission to use photographs and/or video recordings of my child taken during sessions for use in promotional materials, website content (including JTHockey.ca), and social media, without compensation. Note: JTHockey utilizes video analysis as a teaching tool.

Player & Parent Code of Conduct: JTHockey enforces a zero-tolerance policy regarding bullying, harassment, or abuse towards coaches, players, or officials.

• Players are expected to listen, compete, and respect the learning environment.

• Disruptive behavior that negatively impacts the group experience may result in expulsion from the session or program without refund.

Group Placement: I understand that JTHockey reserves the final decision on group placement. If it is determined that a player is better suited for a different group based on skill level or safety, we will offer a transfer. If a suitable group is not available, a full refund will be issued.

JTHOCKEY REFUND POLICY

Because our programs have strict participant caps (20 skaters) and fixed costs (ice/staff), we rely on committed registrations to operate.

1. Withdrawal by Participant:

• 30+ Days Prior to Start Date: Full refund minus a $25 Administrative Fee (covers credit card processing and admin time).

• 15-29 Days Prior to Start Date: 50% Refund or Full Credit toward a future JTHockey program.

• Less than 14 Days Prior to Start Date: No refunds or credits will be issued, except for medical reasons.

2. Medical Withdrawals: If a player is unable to participate due to injury or illness, a pro-rated credit for future programs will be issued upon receipt of a valid doctor's note.

3. Program Cancellation: If JTHockey cancels a session or program for any reason (e.g., facility closure), a full refund for the missed session(s) will be issued automatically.

Note: The original credit card processing fees (approx. 3%) are non-refundable by the payment processor and are not covered by JTHockey.

By registering with TeamLinkt, you commit to adhere to the refund policies, fees, and timelines set by your chosen organization or program. You commit not to initiate any chargebacks.  Deviations from this policy may result in the forfeiture of any refund.
