Welcome to Jr Cougar Football Tackle Registration. Registration is open through August 10, 2024.  This registration form is for students entering 6th-8th grade in the 2024/2025 school year.  Be sure to read each waiver carefully and accept all terms.  Please include all parent/contact emails that need to be added in our team app & have any discount codes you’ve been provided on hand.

Please select the players grade they will enter in August of 2024. This is the grade they will be rising into for the next school year.

Required Documents

Please provide a utility bill that shows your residence in the Chattahoochee High School district.
Please provide a copy of the players birth certificate or passport.
Please upload a physical examination form completed by a physician . A copy is available to download if needed. IMPORTANT NOTE: This is REQUIRED for your player participate in any season PRACTICE or GAME. If you have not yet had a doctor complete this & would like to submit registration, please email your completed physical to jrsec@hoochfootball.net prior to the first practice.

WAIVERS

It is extremely important that equipment distributed at the start of the season is returned as specified in guidelines provided at the completion of the season. If equipment is not turned in as specified at the completion of the season you will be charged up to $150 using the card information provided on the signed authorization collected at pick up.
I understand that Stack Team will be used for all communications and scheduling throughout the season. You will receive an email with an invite to download the app & create an account, if you do not already have one for your provided email. Additionally, weekly emails with important information will sent from jrcomm@hoochfootball.net. Please be sure these emails are not marked as SPAM and are reviewed when received.
To ensure a positive, safe, and respectful environment for all participants, we have established a comprehensive Code of Conduct for both players and parents. Adherence to these guidelines is crucial for the success and integrity of our program. Please review the attached document that provides Parent Expectation and Player Responsibilities.
I understand that all FORMS must be uploaded during the registration process. Forms provided include proof of age & residency, completed physical examination, player safety information, and applicable GMSAA forms. Returning 2023 Fall Season players do not need to resubmit PROOF OF AGE or GMSAA forms. NOTE: If a completed physical exam in not available at the time of registration one MUST be provided before you player can participate in any practice or game.

Our program is committed to providing a high-quality experience for all participants. To achieve this, considerable resources and planning go into preparation long before the season begins. We understand that circumstances change, and there may be instances where participants need to withdraw from the program. Our refund policy is designed to be transparent and fair to all parties involved, taking into consideration the expenses incurred by the program prior to the start of the season.

  • Prior to the First Fall Practice: Participants who withdraw from the program before the start of the first fall practice will be eligible for a full refund. This policy acknowledges the possibility of early changes in participants' plans and aims to minimize financial impact.
  • Prior to the First Season Game (Excluding the Jamboree): For withdrawals that occur after the first fall practice but before the first official season game, refunds will be issued minus a $250 withdrawal fee. This fee is necessary to cover the portion of program expenses already incurred in anticipation of the participant's involvement.
  • After the First Season Game: We regret that no refunds can be issued to participants who withdraw after the first season game has taken place. Substantial program resources have been allocated, and the withdrawal fee would not sufficiently cover these costs.

Please note that the above refund policies apply on a per-player basis and are non-negotiable. By registering your athlete for our program, you acknowledge and agree to this refund policy. We appreciate your understanding and cooperation in ensuring the program's success and sustainability.
