Medical Risk Waiver:

By accepting the terms and conditions you're indicating you understand that soccer is a potentially dangerous activity that I or my child could be killed or seriously injured while participating. Injuries that could occur include but are not limited to: paralysis, brain injury, sprains and broken bones. I understand that if my child is participating in this youth sports program that he/she may be coached by a volunteer or paid coaches who will not be full time and may not be professionally trained. Recognizing the inherent risks associated with participating in the above mentioned program and still desiring myself or my child to participate, I hereby agree to indemnify and hold harmless the kentucky Wolfpack Soccer Club LLC, Owners/volunteers/coaching staff, venue property owners/members, employees and all individuals responsible for the conduct of activities involving myself or my child(ren) for claims including, but not limited to claims of personal injury, hospitalization, etc. I also understand that the above mentioned entities strongly recommend that each participant have medical approval before participating in any soccer related program hosted by Kentucky Wolfpack Soccer Club LLC and that I must inform the club of any medical condition that may require special attention or treatment. I also understand the inherent risks of Covid 19 and any variants as well as other communicable diseases and hold harmless the Kentucky Wolfpack Soccer Club and property owners/members of any property/venue where activities are held.

Photo and Social Media Policy: I understand and give permission for the Kentucky Wolfpack Soccer Club to photograph or record my child or me during participation in club sponsored activities and to utilize them in advertising and or promotion both in print and or on the clubs website and social media. If desired to not be photographed or filmed, written notice must be given to the club.

Emergency Policy: In the event of an emergency, I give my permission for a representative of the club and/or ambulance service to provide treatment as is normal and prudent and to transport my child or myself to the nearest medical facility to render treatment.

Sportsmanship Policy: Unsportsmanlike conduct by any coach, player, parent spectator or other member is strictly forbidden at any game, practice or other Kentucky Wolfpack Soccer Club function. I understand that the referee has the right to eject any coach, player, or parent and to terminate the game for reasons of unsportsmanlike conduct. I understand that any ejection is subject to suspension by the Kentucky Wolfpack Soccer Club director/assistant director.
