KFRec: Youth Flag Football

I, named participant(s) or guardian of participant(s) listed above, for the Ascension Parish Kingdom Fitness Flag Football League/Tournament give my approval for participation in any & all activities, including transportation to & from the facility/field. This includes walking & auto transport. All players, coaches, & recognized team supports must follow established rules & codes of conduct policy as instructed by staff for the safety of all participates. I, know that participation in this fairly contact event can result in serious injury or injuries & any protective equipment does not prevent injuries from occurring. Therefore, I herby waive, release, absolve, indemnity & agree to hold harmless the local facility/field, Ascension Parish, Kingdom Fitness sponsors, supervisors, participants, referees, staff, & persons from activities for any injury to me, my child or my guest whether the result of negligence or any other cause. In addition I drive & park my own transportation vehicle at my own risk while event is going on. I hereby consent for the authorized medical treatment & signed hospital forms required to permit any hospital treatment, if accident happens. I, hereby consent & release for any photographs or videos made by any party during participation. I give consent for Kingdom Fitness Recreation to use the photographs taken for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and/or web content. Furthermore I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

The policy of Kingdom Fitness Recreation is to issue NO REFUNDS. It is to the discretion of the board of the organization to overturn the NO REFUNDS policy in any and all situations that may arises before the start of the season where a player/team can no longer participate. Kingdom Fitness Recreation reserves the right to cancel any division based upon low enrollment and or conditions beyond our control. In the event Kingdom Fitness Recreation cancels a division, the participant/team will be offered the option to move to another division posing no harm or unfairness. NO REFUNDS WILL BE GIVEN! NO REFUNDS will be considered after the close of registration for any reason.
