1. All Dates for Scheduled Events are subject to change.
2. Code of Conduct: ll players will represent Lake Orion and Friends of Lake Orion Lacrosse and abide by the Code of Conduct as outlined by LOHS Student Handbook pages 25-34 Athletes expected to be Drug, Alcohol, Smoke/Vape free. Failure to comply will result in removal from the participation
3. Photo Release: I hereby grant permission to use photographs and/or video of the player I am registering at any organization sponsored event in publications, news releases, social media, online, and in other communications related to Lake Orion High School Lacrosse or Friends of Lake Orion Lacrosse
4. Risk Acknowledgement: I, the undersigned parent/guardian of the player I am registering, hereby give my consent for my child to participate in the Lake Orion / Friends of Lake Orion Lacrosse Club. I acknowledge that lacrosse is a contact sport that involves inherent risks of physical injury. I have discussed these risks with my child, and we both understand and accept them. I affirm that my child is physically fit and mentally capable of participating in lacrosse activities, and I have not been advised otherwise by a qualified medical professional.
5. Liability Waiver: I hereby release, waive, discharge, and covenant not to sue Lake Orion High School or the affliliated booster and club organization known as Friends of Lake Orion Lacrosse, its directors, board, officers, employees, coaches, volunteers, agents, and other participants (collectively 'Releases') from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death, that may be sustained by my child or to any property belonging to my child, while participating in such activity, or while on the premises where the event is taking place.
6. Medical Authorization: In the event of an injury to my child while participating in the club activities, I give my consent to Lake Orion High School or r the affliclated booster and club organization known as Friends of Lake Orion Lacrosse to seek, through a physician of their own choice, medical or surgical treatment if deemed urgently necessary and appropriate on my child's behalf.
7. Acknowledgment: By continuing with this registration I affirm that I have read this Waiver and Release of Liability Agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.'
8. Equipment Responsibility: Players are responsible for providing all required neccesary Lacrosse Equipment as defined by USA Lacrosse
