Requirements outlined below to ensure you are prepared for game day.

Background Screening/APS Training
- All adults listed on rosters at JVA insured events must complete the Background Screen and APS training unless they have a USAV screen and Safe Sport certification.
Participation Release and Liability & Medical Release/Waiver Forms
- These forms are required to be with coaches/managers at all times
- Forms (hard or electronic copies) from other associations are accepted.
Coaches Event Sign-In Form needs to be submitted by all teams. You will submit this during registering for the event on our site.
