Once you have completed registration, you will receive a link to the AYF Player Package pre-populated forms. Please print and sign all forms. Be sure to complete any applicable information which may have been left blank.

You will also need to submit the following which are not included in the link:

2 copies of your athlete's FINAL Report Card (must include ALL terms of the current school year)
New athletes will need to provide their Original Birth Certificate & Copy of the Birth Certificate. You will be given back the original once certified at Paperwork Night, copy will be kept. Returning players from last season do not need to bring a birth certificate
AYF / AYC Absentee Form - copies will be provided if not included in link. The form should be signed but not dated.
