Please print and sign all forms at the link in this email. Please do not print double-sided. Be sure to complete any applicable information which may have been left blank. We will provide more detailed instructions prior to paperwork night.

There are two options to meet the medical clearance requirement (page 7).

Provide a physical report with a physical visit in 2025 (the actual date of the physical appointment must be / have been in 2025) OR
Submit the Medical Clearance Form to a physician to sign and date

Please plan accordingly with your physician so that your paperwork is ready on our paperwork collection night.

You will also need to submit the following which are not included in the link:

1 copy of your athlete's FINAL Report Card (must include ALL terms of the current school year)
New athletes will need to provide their Original Birth Certificate & Copy of the Birth Certificate. You will be given back the original once certified at Paperwork Night, and the copy will be kept. Returning players from last season do not need to bring an original birth certificate.
