CODE OF CONDUCT AND ANTI-HARASSMENT POLICY
I agree to treat all players, teammates, opponents, organizers, and staff with respect, both on and off the court, regardless of their skill level or experience. I understand that this is a recreational, self-officiated league that people join to have fun. I understand that the following conduct is strictly prohibited:
- Unwanted physical contact of any kind, including touching, grabbing, groping, or deliberately invading another person's personal space.
- Harassment, intimidation, or discriminatory, demeaning, or sexually suggestive comments, gestures, or behavior.
- Aggressive, hostile, or demeaning conduct toward other players, including yelling at, berating, or repeatedly arguing with others over gameplay or self-officiated calls. Disagreements are settled calmly and respectfully.
- Unwanted or persistent contact with another participant outside of league play, through any channel, after that person has not welcomed it or has asked me to stop.
- Any other behavior that a reasonable person would find threatening, hostile, or offensive.
This standard applies at league events and to any conduct connected to my participation, including contact with other participants outside of scheduled play. I may report a concern confidentially to matt@mattsvolleyball.com; reports are handled discreetly, and retaliation against anyone who reports in good faith will not be tolerated.
If the league determines, at its sole discretion, that I have violated this policy, it may take any action it considers appropriate, up to and including immediate removal from the league and a permanent ban from future participation, with no obligation to provide a refund. I agree to comply with any conduct decision the league makes.
ACCEPTANCE OF LEAGUE RULES
I have read the league rules, and agree to follow the posted rules, even if I disagree with said rules.
LEAGUE COMMUNICATIONS
I agree that Matt's Volleyball organizers and night directors may contact me at the phone number and email address I provide during registration for league-related purposes, including schedule and game-day updates, weather cancellations, roster and substitute coordination, RSVP checks, and registration or payment matters. Contact may be by phone call, text message, or email, and message and data rates may apply. I can ask to stop receiving these communications at any time by emailing matt@mattsvolleyball.com, and I understand that opting out may limit the league's ability to notify me of schedule changes or cancellations.
ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby assume all of the risks participating in Volleyball at Saeed's Bar & Grill, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of possible liability without fault.
I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in Volleyball at Saeed's Bar & Grill. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of the Volleyball at Saeed's Bar & Grill in which I may participate and that it will govern my actions and responsibilities at said Volleyball at Saeed's Bar & Grill.
In consideration of my application and permitting me to participate in Volleyball at Saeed's Bar & Grill, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
- I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from Volleyball at Saeed's Bar & Grill. THE FOLLOWING ENTITIES OR PERSONS: ALL associated with Saeed's Bar & Grill, and/or their coaches, agents, representatives, or volunteers.
- I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in Volleyball at Saeed's Bar & Grill, whether caused by negligence or otherwise.
I acknowledge that Volleyball at Saeed's Bar & Grill may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration.
I consent and agree that Volleyball at Saeed's Bar & Grill and/or their coaches, agents, representatives or volunteers may take photographs or digital recordings of me as a participant during this event and use these in any and all media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims or interest and I understand that there will be no financial or other remuneration.
The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.
