We are excited to announce we are now accepting registration forms for our 2026 Vipers Spring Tackle Football Programs!

  • Our Spring Tackle Program is for Grades 5-6 (M/F), Grades 7-8 (M/F) and Grades 9-11 (Males only).
  • Girls grade 9 are able to join tackle football grades 7-8
  • Stay tuned for the Female Flag Football program for grades 8-12.

* All early bird registrations before APRIL 7TH will have their names put into a draw and one lucky winner per team will receive their 2026 Spring Football fees back*

REGISTRATION FORM INFO:

To encourage enrollment of younger players we changed registration fees a bit this year;

  • Grade 5-6   $200
  • Grade 7-8   $250
  • Grade 9-11   $300
  • All registrations Includes $50 worth of raffle tickets, you keep the money from selling tickets
  • ALL FEES ARE DUE ON OR BEFORE THE FIRST PRACTICE (No payment = No equipment). 
  • PARENT MEETING: During the first week of practice

  • OFFLINE PMNT: Cash or Cheque (payable to: Melville Minor Football) - IN AN ENVELOPE WITH PLAYER NAME !!
  • OFFLINE PMNT: E-Transfer to melvilleminorfootball@gmail.com. Be sure to include your PLAYERS NAME in the memo line.

Equipment Deposit $100
Equipment deposit is payable by Cheque or Cash.Cheques will be destroyed after the season. If sending CASH put it in an envelope with the PLAYER NAME, TEAM AND ‘CASH DEPOSIT’ on the envelope. EQUIPMENT DEPOSITS AND ANY OUTSTANDING FEES WILL BE COLLECTED ON THE FIRST DAY OF PRACTICE ONLY. 

Epuipment deposit cheques will be cashed if equipment is missing, damaged, or returned late.
Equipment Fitting will be held during the first week of practice

IMAGE/VIDEO CONSENT: By registering in the program consent is given to use images/videos of players on social media and/or MMF website. Consent can be revoked at anytime in writing to MMF.

PLAYERS ARE NOT TO HAND IN ANYTHING TO COACHES. THERE WILL BE A PARENT COLLECTING OUTSTANDING FEES AND FORMS ON THE FIRST DAY OF PRACTICE AND THE PARENT MEETING. PLEASE PUT IN AN ENVELOPE OR ZIPLOC BAG LABELLED WITH PLAYERS NAME.

MELVILLE MINOR FOOTBALL HEALTH WAIVER FORM

 

I am confident that my son/daughter is healty to participate in the sport of footbal. I permit him/her to participate in the physical activity that will take place leading up to and including scrimmages and games for the current season. 

I also agree to have my son/daughter be examined by a doctor after an illness or injury to re-estrablish the bill of good health, and that any medical examinations are my sole responsibility. We, as organizers, require the parent(s) to acknowledge the health of their child in order to participate. 

 

I have answered the medical questions to the best of my knowledge during this registration process.
