Miracle League of Martin County

In consideration for The Miracle League of Martin County providing the opportunity for me, my family, and those I register in this Account to participate in Miracle League baseball on behalf of his or herself, his or her relatives, those registered in this Account, and the heirs, executors, administrators, and assigns of each of them, does hereby release and agree to indemnify and hold harmless the Miracle League of Martin County, it’s staff, officers and directors, and volunteers, from any and all claims for personal injury, death, property damage, or any type of claim or damage (including but not limited to attorney’s fees or litigation expenses) resulting from any activities in connection with participation in Miracle League of Martin County baseball. I consent for me, my family member, and those I register under this Account to receive first aid and/or emergency medical care in the event of an injury.

I understand that there will be media and promotional coverage of the Miracle League of Martin County games and activities and I consent to publish images of me, any family member and anyone else registered in this account and to use our names for such purposes. I represent I am authorized to provide the foregoing consent on behalf of those registered with this account. 

Miracle Leage of Martin County is for players with a medical condition that prohibits or greatly limits them from competing in a typical competitive league/program. 
The Miracle League of Martin County reserves the right to make a final determination on participation in the league. 

All parents or legal guardians of participants are required to be available during entirety of games (unless participants are 18 and older and/or are their own legal guardian) and are to be a positive support for a safe and fun environment in the stands.
