Waiver and Release of Liability I, the undersigned parent or legal guardian, grant permission for my child to participate in the Children’s Ramadan Program (the "Program"). I acknowledge and agree to the following terms:

Assumption of Risk & Release of Liability
I understand that participation in the Program involves various activities that may include but are not limited to arts and crafts, group games, and educational sessions. I acknowledge that these activities may carry inherent risks, including but not limited to minor injuries such as cuts, bruises, or allergic reactions. I voluntarily assume all risks and hereby release, discharge, and hold harmless the Program organizers, volunteers, staff, and facility owners from any and all claims, liabilities, demands, or causes of action arising out of my child’s participation in the Program.
Medical Authorization
In the event of an emergency, I authorize the Program staff to seek necessary medical attention for my child. I understand that the organizers will make reasonable efforts to contact me or the emergency contact provided before seeking treatment. I accept full financial responsibility for any medical expenses incurred.
Allergies & Special Needs
I understand that it is my responsibility to inform the Program organizers of any allergies or medical conditions my child has. While reasonable precautions will be taken to prevent allergic reactions, I acknowledge that the Program organizers are not responsible for any accidental exposure or reactions that may occur.
Photo/Video Release
I grant permission for my child’s photographs or videos taken during the Program to be used for promotional, educational, or informational purposes by the Program organizers. This may include but is not limited to social media, websites, brochures, or newsletters. If I do not consent to this, I will provide written notification to the Program organizers.
Behavioral Expectations & Dismissal
I acknowledge that my child is expected to follow Program rules and instructions from staff. If my child exhibits disruptive or unsafe behavior, I understand that I may be contacted to pick them up, and they may be dismissed from the Program without a refund.
Property Damage Liability
I accept full responsibility for any damages caused by my child to the facility, equipment, or other participants' belongings, whether accidental or intentional. I agree to reimburse the Program for any repair or replacement costs.
Program Changes & Cancellations
I understand that the Program organizers reserve the right to modify or cancel the Program due to unforeseen circumstances. Any changes will be communicated to parents/guardians in a timely manner.
