This membership covers the insurance to play the sport (insurance policy protects participants, bystanders, and property from injury). I fully understand that the NSI Lacrosse program is for players residing in the attendance boundary for Newport/Sammamish/Interlake High School. The only exceptions are for those players who: 1. Their home public high school is Newport/Sammamish/Interlake HS, but attend International School. 2. Their home public high school is Newport/Sammamish/Interlake HS, but attend Big Picture School. 3. They are enrolled in another BSD choice program (e.g. ISA) that feeds Newport/Sammamish/Interlake High School. 4. Their home public high school is Newport/Sammamish/Interlake HS, but they are home schooled or attend a private high school that does not offer lacrosse as a sport.

Concussion Info

On May 14, 2009, the Washington State Legislature passed the "The Zachary Lystedt Law" HB 1824, directing the education of coaches, players, and parents on the recognition and management of concussions as well as establishing return to play protocol for all youth athletes suspected of or having a concussion or brain injury. What this means for you is that all Parents/Guardians shall annually review, sign and return to the school, a concussion and head injury information sheet prior to the youth athlete's initiating practice or competition. Please read and complete at your earliest convenience to ensure the safety of your player. A Fact Sheet for Parents and Players WHAT IS A CONCUSSION? A concussion is a brain injury. Concussions are caused by a bump or blow to the head. Even a “ding,” “getting your bell rung,” or what seems to be a mild bump or blow to the head can be serious. You can’t see a concussion. Signs and symptoms of concussion can show up right after the injury or may not appear or be noticed until days or weeks after the injury. If your child reports any symptoms of concussion, or if you notice the symptoms yourself, seek medical attention right away. WHAT ARE THE SIGNS AND SYMPTOMS OF A CONCUSSION? Signs Observed by Players or Guardians If your child has experienced a bump or blow to the head during a game or practice, look for any of the following signs and symptoms of a concussion: Appears dazed or stunned Is confused about assignment or position Forgets an instruction Is unsure of game, score, or opponent Moves clumsily Answers questions slowly Loses consciousness (even briefly) Shows behavior or personality changes Can’t recall events prior to hit or fall Can’t recall events after hit or fall Symptoms Reported by Athlete Headache or “pressure” in head Nausea or vomiting Balance problems or dizziness Double or blurry vision Sensitivity to light Sensitivity to noise Feeling sluggish, hazy, foggy, or groggy Concentration or memory problems Confusion Does not “feel right” HOW CAN YOU HELP PREVENT A CONCUSSION? Every sport is different, but there are steps you can take to protect yourself from concussion. Ensure that you follow your coach’s rules for safety and the rules of the sport. Practice good sportsmanship at all times. Always wear the right protective equipment you are issued (such as helmets, padding, and eye and mouth guards). Protective equipment should fit properly, be well maintained, and be worn consistently and correctly. Learn the signs and symptoms of a concussion. WHAT SHOULD YOU DO IF YOU THINK YOUR CHILD HAS A CONCUSSION? Seek medical attention right away. A health care professional will be able to decide how serious the concussion is and when it is safe for your child to return to sports. Keep your child out of play. Concussions take time to heal. Do not return your child to play until a health care professional says it’s OK. Children who return to play too soon—while the brain is still healing—risk a greater chance of having a second concussion. Second or later concussions can be very serious. They can cause permanent brain damage, affecting your child for a lifetime. Tell your Coach about any recent concussion. Coaches should know if your child had a recent concussion in ANY sport. Your coach may not know about a concussion your child received in another sport or activity unless you tell the coach.

My child and I have read the Parent/Player Fact Sheet on CONCUSSION IN YOUTH SPORTS on this website, and I understand our responsibility to report any symptoms to the coach immediately and not to return to play until my child obtains written clearance from a licensed health care provider trained in the evaluation and management of brain injuries
Parental Consent, Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement All Participants must Check the Electronic Signature box indicating that you have read this document. IN CONSIDERATION of my minor child ("the Minor") being permitted to participate in certain lacrosse activities (“activities”) sponsored or organized by Newport Lacrosse (NL), minor participant, including parent(s) of minor participants agree as follows: 1. Playing the sport of lacrosse involves serious risks. The activities involved in the sport vary but can include throwing, running, catching, passing and checking (hitting with body and/or stick). The potential hazards and dangers associated with the sport include, but are not limited to: being hit by the ball, colliding with another player, being hit with a lacrosse stick. These hazards and dangers may result in fractures, sprains, broken bones, concussions, cuts or abrasions, paralysis, disability or death. Some risks are inherent in these activities and cannot be eliminated or reduced. 2. I FULLY UNDERSTAND and will instruct the Minor participant that: (a) THE SPORT OF LACROSSE IS POTENTIALLY VERY DANGEROUS and such use involves RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH; (b) these Risks and dangers may be caused by the Minor's own actions, or inactions, the actions or inactions of others, the condition and layout of the premises and equipment, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS NOT KNOWN TO ME or that are not readily foreseeable at this time; (d) THE SOCIAL AND ECONOMIC LOSSES and/or damages that could result from those Risk(s) COULD BE SEVERE AND COULD PERMANENTLY CHANGE THE MINOR'S FUTURE. 3. I consent to the Minor's participation in the activities and HEREBY ACCEPT AND ASSUME ALL SUCH RISKS, KNOWN AND UNKNOWN, AND ASSUME ALL RESPONSIBILITY FOR THE LOSSES, COSTS AND/OR DAMAGES FOLLOWING SUCH INJURY, DISABILITY, PARALYSIS OR DEATH, EVEN IF CAUSED, IN WHOLE OR IN PART, BY THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW. 4. I HEREBY RELEASE, DISCHARGE AND COVENANT NOT TO SUE Newport Lacrosse, its officers, directors, agents, coaches, members, employees, attorneys, consultants and other persons or entities who give recommendations, directions, or instructions or engage in risk evaluation or loss control activities regarding the activities and each of them, all for the purposes herein referred to as "Releasees," FROM ALL LIABILITY TO ME, THE MINOR, and the minor's personal representatives, assigns, heirs, and next of kin, FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON ACCOUNT OF ANY INJURY TO ME OR THE MINOR, including, but not limited to, death or damage to property, CAUSED OR ALLEDGED TO BE CAUSED, IN WHOLE OR IN PART, BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE. This release and indemnity agreement is intended to include any losses claimed including but not limited to claims for personal injury, property damage, wrongful death, products liability, breach of contract or otherwise. 5. If, despite this release, I, the Minor, or anyone on the Minor's behalf, makes a claim against any of the "Releasees" named above, I AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS THE RELEASEES and each of them from ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS, LIABILITY, DAMAGE, OR COST THEY MAY INCUR DUE TO THE CLAIM MADE AGAINST ANY OF THE "RELEASEES" NAMED ABOVE, WHETHER THE CLAIM IS BASED ON THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. 6. The Minor participant has no mental or physical problems that might compromise or affect his/her ability to participate in lacrosse activities which have not been disclosed to Newport Lacrosse. 7. I agree that this Consent, Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement shall be governed by the laws of the State of Washington. Any portion of this document deemed to be unlawful or unenforceable shall not affect the enforceability of the remaining provisions which shall remain in full force and effect. I authorize Newport Lacrosse staff to obtain or provide medical care for the Minor participant or to transport the Minor to a medical facility. I authorize medical personnel to render such treatment they deem necessary for the Minor’s health. I agree that Newport Lacrosse has no responsibility for medical care related expenses provided to the Minor and I agree to pay all costs associated with such care and transportation. I HAVE READ THIS PARENTAL CONSENT, RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, UNDERSTAND THAT BY SIGNING IT I GIVE UP SUBSTANTIAL RIGHTS I AND/OR THE MINOR WOULD OTHERWISE HAVE TO RECOVER DAMAGES FOR LOSSES OCCASIONED BY THE RELEASEES' FAULT, AND SIGN IT VOLUNTARILY AND WITHOUT INDUCEMENT.
Newport Lacrosse Code of Conduct Coaches 1. Coaches will always be fair, firm and consistent. 2. Coaches will promote a positive attitude and lead by example. 3. Coaches will stress teamwork and respect with each and every athlete. 4. Coaches will listen to their players concerns and try to help them any way they can. 5. Use of profanity, drugs, alcohol or tobacco during any Newport Lacrosse event (including practice) is prohibited. 6. Coaches will allow each athlete the opportunity to compete and excel. 7. Coaches must submit to a Washington State background check before allowed to coach, a copy must be on file with the Board of Directors. 8. Coaches will strictly adhere to the policies and procedures of GELL and/or WHSBLA, as applicable. 9. Coaches will not engage in inappropriate conduct of any sort, or verbal abuse towards any player, parent, or referee. 10. The Board of Directors, in its sole discretion, reserves the right to suspend, terminate or expel, any coach who fails to comply with this Code of Conduct. Parents / Guardians 1. Parents will comply with this Code of Conduct and all rules established by GELL and the WHSBLA. 2. Parents should set the right example for players by always showing good sportsmanship. 3. Parents will not argue with a coach, referee or official at any event. 4. Use of profanity, drugs, alcohol or tobacco during any event is prohibited. 5. Any parent or spectator who crosses the field barriers during a game risks their player being disqualified from the game, Newport Lacrosse and/or GELL. Parents and spectators are to be situated on the opposite side of the field from the player’s box (where applicable). 6. Parents who incur an un-sportsmanlike behavior penalty put their player at risk of disqualification or from further participation. 7. Understand that your player will be given every opportunity to participate; however, the safety of our athletes is our #1 concern. 8. Your player must meet GPA requirements of the BSD athletic department in order to play plus any specific GPA requirements of our coaching staff. 9. The Board of Directors, in its sole discretion, reserves the right to take the following actions against any parent(s) who fails to comply with this Code of Conduct including but not limited to expulsion or suspension fro Newport Lacrosse and/or denial of access to practices/games. Players 1. Players will try their very best to support their team. 2. Players will be team players and always put the team first. 3. Players will stay within the team areas of the sidelines during games. 4. Use of profanity, drugs, alcohol or tobacco during any event is prohibited. 5. Players will listen to their coaches and agree to follow the rules established by GELL and the WHSBLA, as applicable. 6. Players will demonstrate good sportsmanship on and off of the field of play and will not be malicious or retaliate with their lacrosse stick. 7. Players will attend all practices, games, meetings and any other team function on time. 8. Players will show respect towards their teammates, coaches, members of the opposing team and referees / officials at all times. 9. Players will not fight, use foul language or argue with coaches, teammates, or referees /officials. 10. Players must meet GPA requirements of the BSD athletic department in order to play plus any specific GPA requirements of our coaching staff. 11. The Board of Directors reserves the right to take the following actions including but not limited to Newport Lacrosse membership suspension, expulsion or denial of access to practices/games against any player who fails to comply with this Code of Conduct. All 1. Unbecoming conduct, including abusive language and/ or gestures, hitting, racially insensitive remarks, use of drugs or alcohol, damage to property or belongings, or any other conduct that reflects poorly on the club or lacrosse in general is unacceptable and will not be tolerated. This rule applies to all players, parents, coaches, siblings and guests at all times when in or around games or practices as a member of Newport Lacrosse. 2. During away games or tournaments, you represent Newport Lacrosse. Unbecoming conduct, including the examples described above, will not be tolerated on or off the field. This rule applies in full force for away games or tournaments. ANY SUSPENSION OR EXPULSION BASED ON VIOLATIONS OF THE CODE OF CONDUCT WILL NOT BE ACCOMPANIED BY A REGISTRATION REFUND, IN WHOLE OR PART.
By registering your player for the season, you (parent/guardian) agree to participate as a volunteer in some capacity during the season. Newport Lacrosse is a non-profit organization funded by registration fees and donations. Financial support, however, is only part of the picture. The Club's "people power" helps to make it run! That's where you come in. The club will distribute a roster and schedule with roles to be filled.
Photograph & Video Release Liability Waiver Newport Lacrosse currently has a website, Facebook, Instagram and Twitter account. We are proud to show off the accomplishments and achievements of our athletes and club. When posting pictures and videos online, we will only use the athlete’s first name or screen name. I grant consent to Newport Lacrosse to identify a picture or video of my child or ward, by first name, in any Newport Lacrosse sponsored material, publication or video. This consent is valid until I (guardian), ask the President or Webmaster of Newport Lacrosse to revoke, at any time, this consent to post pictures and videos online.
Waiver/Release for Communicable Diseases Including COVID-19 In consideration of being allowed to participate in a Newport Lacrosse event (“NLAX Event”), the undersigned acknowledges, appreciates, certifies and agrees that: 1. My participation includes possible exposure to and illness from infectious diseases, including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness, injury, and death does exist. 2. If I have a pre-existing health condition, exposure to COVID-19, or any other infectious disease may be more likely to cause serious illness, injury, or death; 3. NLAX cannot ensure that all other participants, including coaches and volunteers, are taking precautionary measures to mitigate risks to ensure the health and safety of other participants, coaches, and volunteers, and therefore, participation in a NLAX Event involves risk of exposure to infectious disease; and, 4. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 5. I certify that I have not recently tested positive for, and am not exhibiting symptoms of COVID-19, which include a cough, shortness of breath or difficulty breathing, loss of taste or smell, headache, chills, muscle or body aches and/or sore throat. 6. I certify that I do not have a household family member/roommate who has recently tested positive for or exhibited the above-referenced symptoms of COVID-19. 7. I willingly agree to comply with all recommendations provided by NLAX to ensure safe play. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest coach, staff member or volunteer, or official immediately; and, 8. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS NLAX, and their officers, officials, agents, and/or employees, other participants, volunteers, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION) This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

Newport Lacrosse fees are generally NOT refundable:

REGISTRATION fees are NON-REFUNDABLE if a player is placed on a team but it’s not the team he wanted (e.g. made JV but wanted Varsity). This also applies to youth programs and all other team divisions. 

REGISTRATION fees are NON-REFUNDABLE if a player quits the team after the season has started.

TOURNAMENT fees are NON-REFUNDABLE.

TRAVEL fees are NON-REFUNDABLE.

Falsified information regarding player eligibility will result in a forfeit of all fees paid.

Newport Lacrosse will only process a refund in the following cases:
1. Severe hardship or medical issue reported BEFORE first practice.
2. Financial aid application is declined and refund requested BEFORE first practice.

REFUNDS ARE SUBJECT TO A 5% CHARGE FOR CREDIT CARD FEES.
