Informed Consent/General Release - Youth Sports Participants

This is a release of liability. Please read carefully before signing.
Since participation in youth sports activities can be dangerous, Odessa Soccer Club requires all participants (and their adult parents(s) or guardians) to assume all risks associated with youth sports by signing this general release.
For and in consideration of my child being permitted to participate in Odessa Soccer Club youth sports activities, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, infection due to communicable disease (including all bacterial and viral infections), death, or property damage which I or my child may have, or which may hereafter accrue to me or my child, as a result of my participation in youth sports activities during play and while I am at the facility while others play or for any other reason. This release is intended to discharge, in advance, Odessa Soccer Club, its officers, employees and agents, and the owners and maintainers of any facility or field used for the activities, from any and all liability arising out of or connected in any way with my child’s participation in sports camp/clinic activities, even though that liability may arise out of negligence or carelessness on the part of Odessa Soccer Club, its officers, agents or employees, or the owners and maintainers of any facility or field used for the activities.
I further understand that serious accidents occasionally occur during youth sports activities, and that participants occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my child to participate in the activity and thereby agree to assume those risks to release and hold harmless Odessa Soccer Club, its officers, employees or agents used for the activity, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages. I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child’s heirs, executors, administrators and assigns. I further agree to indemnify and hold harmless Odessa Soccer Club, its officers, employees and agents for any loss, liability, damage, cost or expense which they may incur as a result of any injury or property damage I or my child may sustain while participating in the activity. I agree to comply with the program’s stated and customary terms and conditions for participation according to Odessa Soccer Club. If I observe any significant changes with regard to my child’s readiness for participation in the program, I will remove my child from the program immediately. I have read this Informed Consent/General Release fully understand its terms, that I give up substantial rights by signing it, and sign it voluntarily. This document is a Release of Liability which affects the rights of you and your child. Please read the document carefully before signing.
I have read this Informed Consent/General Release and I understand the seriousness of the risks involved in participating in this program, my personal responsibilities for adhering to rules and regulations, and accept them as a participant. (To be signed by all players who are league age 12 or older.)

Parental Pledge
Odessa Soccer Club holds its parents to a very high standard of behavior. Odessa Soccer Club's primary purpose is to develop players in character and athleticism. In order to achieve this goal, parents must support and contribute to the club in these efforts.
Odessa Soccer Club requires all parents to agree to the following Sports Parents Pledge:
I pledge to promote positivity in youth sports and act as a role model to my child, as well as other participants and sports parents I meet. I will serve as an advocate for good sportsmanship and to keep fun and learning at the center of the youth sports experience. I will try to improve upon my youth sports knowledge so that I can be an informative leader in the youth sports community. I understand that behavior deemed contradictory to the values taught by Odessa Soccer Club, the FIFA Laws of the Game, or rules adapted to Odessa Soccer Club may result in me being asked to leave the premises.

Pre-game Prayer
Since Odessa Soccer Club's primary purpose is to develop players in character and athleticism, a pre-game prayer will be led by the League Director. This prayer will be general in nature and will be inclusive to all walks of faith in God. Players of families who do not participate in prayer are not required to pray and can observe moments of silence instead.

Spectator Area
Coaches/Players and Families have designated areas where they must be during games. Part of player development is to build players to be independent and make independent decisions during games. The designated Coaches/Players and Family areas will be identified with signs. Family members must not watch games or remain in the areas designated for Players/Coaches.

Social Distancing
Players and spectators will maintain knowledge of prevailing Social Distancing requirements during all Odessa Soccer Club events. At their discretion, players, when not on the field during a game, and spectators will maintain a six (6) foot distance from one another.

COMMUNICABLE DISEASE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
In consideration of being allowed to participate in any way in any Odessa Soccer Club Inc. (“OSC”) related events and activities I, the undersigned participant, parent, or legal guardian, acknowledge, appreciate, and agree that:
By participating in OSC related events and activities, there are certain risks to me arising from or related to possible exposure to communicable diseases including, but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for the Coronavirus Disease (also known as COVID-19) and/or any mutation or variation thereof (collectively referred to as “Communicable Diseases”). I am fully aware of the hazards associated with such Communicable Diseases and knowingly and voluntarily assume full responsibility for any and all risk of personal injury or other loss that I may sustain in connection with such Communicable Diseases.
I, for myself or for my minor child(ren) or ward(s), and on behalf of my/our heirs, assigns, beneficiaries, executors, administrators, personal representatives, and next of kin, HEREBY EXPRESSLY RELEASE, HOLD HARMLESS, AND FOREVER DISCHARGE ODESSA SOCCER CLUB INC. and its officers, officials, agents, representatives, employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises upon which OSC related events and activities take place (the “Released Parties”), from any and all claims, demands, suits, causes of action, losses, and liability of any kind whatsoever, whether in law or equity, arising out of or related to any ILLNESS, INJURY, DISABILITY, DEATH, OR OTHER DAMAGES incurred due to or in connection with any Communicable Diseases, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES OR OTHERWISE, to the fullest extent permitted by law.
I agree that this Agreement is intended to be as broad and inclusive as is permitted by the laws of the State of Florida, and if any portion hereof is held invalid, it is agreed that the remainder shall continue in full legal force and effect.

COVID-19 SAFETY PROTOCOLS
Odessa Soccer Club considers the health and well-being of our players, families, and our community its top priority. To this end, we recommend you consider adhering to the following procedures to help mitigate the spread of COVID-19. We implore you to take these measures seriously and work with us to ensure they are followed each week.
- It is optional for Players and Coaches to wear masks during practices and games.
- If your player is or has been sick or symptomatic, please do not attend.
- If your player has been in close contact with someone who has been sick or symptomatic, please do not attend.
- Wash hands thoroughly and often before arriving at the field.
- Use hand sanitizer (60% Alcohol) while at the field.
- A Large hand sanitizer dispenser will be available at each field.
- Players and families may also bring their personal hand sanitizers
- Avoid shaking hands/high fives, etc. - but great each other in a non-contact way and SMILE a lot!
- Cover mouth and nose and use a tissue when sneezing/coughing.
- Do not spit.
- Players should only handle their personal ball during practices.
- Balls should be disinfected before/after training and games.
- Remind players not to share water bottles or food.
- If snacks are provided by a coach or parent, they should be single-serving, packaged items.
- Parents should consider dropping off players or remain in car during practice and games.
- Adults and Players who are not active in the game and practice should consider practicing Social Distancing.
- Teams will not participate in a post-game handshake or high five.
Parents, Social Distancing and COVID-19 Precautions are ever-changing. It is your responsibility to be aware of and comply with prevailing local, community, state and otherwise COVID-19 rules and practices.
We are counting on the parents to adhere to these guidelines and to ensure your players do as well.
