Old Pueblo Football – Participant Terms, Waiver and Release (Winter 2027)
Old Pueblo Football League, LLC (“Old Pueblo Football” or “OPF”). By completing this registration, the parent or legal guardian (“I”) agrees to the following on behalf of myself and the registered participant (“my child”). If the participant is 18 or older, “my child” means the participant, who also agrees to these terms.
1. Code of Conduct
OPF holds athletes, parents, coaches and spectators to a high standard of sportsmanship. Violations may result in disciplinary action against the violator and/or the associated athlete, up to suspension or permanent removal from the league without refund. The League Representative decides the length and severity of any discipline.
Players
- I will show good sportsmanship to players, coaches, officials, opponents and parents, win or lose.
- I will be honest, fair and respectful, encourage my teammates, and arrive on time and ready to play.
Parents and spectators
- I will put the emotional and physical well-being of every player ahead of winning.
- I will respect players, coaches, officials and other families, and accept the decisions of officials and coaches.
- I will not use profanity, threats, violence, or yell instructions to my child during games.
- I will not belittle or ridicule anyone involved in youth sports, in person or online.
Zero tolerance (all participants and spectators): fighting, threats, harassment, discrimination, or bringing weapons, alcohol, drugs, tobacco or vapes to any OPF event. Violators may be removed from the facility immediately.
2. Required Safety Equipment
All players must wear soft-shell headgear in every game (Big Man Rush 5v5: full-coverage soft-shell helmet; 7v7: padded ring-style headgear or full-coverage soft-shell helmet). Hard-shell helmets are not allowed and headgear may not be shared. A mouthpiece is required for all players. The 7v7 quarterback is exempt from headgear and mouthpiece only while playing QB. Headgear is available from OPF or any retailer. No approved headgear, no play. Headgear reduces but does not eliminate the risk of head injury.
3. Concussion and Head Injury Acknowledgment
- I understand football, including flag and 7v7, carries a risk of concussion and other head injuries, which in rare cases can cause permanent brain damage or death.
- I have reviewed, or will review, the CDC HEADS UP concussion information (cdc.gov/headsup) with my child.
- Any player who shows signs of a concussion will be removed from play and may not return that day. Before returning, the player must provide written clearance from a licensed health care provider.
- I will tell OPF and my child's coach about any prior concussion or medical condition that affects my child's safety.
4. Medical Authorization and Health
I confirm my child is physically able to participate. A sports physical is not required but is strongly recommended. If I cannot be reached in an emergency, I authorize OPF, its coaches and volunteers to obtain first aid and emergency medical treatment for my child, including transport to a medical facility. I am responsible for all medical costs. OPF does not provide medical or accident insurance for participants.
5. Assumption of Risk, Release of Liability and Indemnification
I understand that football activities involve inherent risks, including collisions, falls, heat illness, and injuries ranging from minor to serious, including paralysis and death. I voluntarily assume these risks for myself and my child.
To the fullest extent permitted by law, I release, discharge and agree not to sue Old Pueblo Football League, LLC; its owners, officers, coaches, officials, employees and volunteers; and the owners and operators of any facility used for OPF events, including Pima County, the Pima County Stadium District and Kino Sports Complex (together, the “Released Parties”). This applies to any claim for injury, illness, death or property damage arising from my child's participation in or attendance at OPF activities, including claims caused by the ordinary negligence of the Released Parties.
I agree to indemnify and hold harmless the Released Parties from any claim, including reasonable attorney's fees, brought by me, my child or anyone on our behalf arising from my child's participation.
6. Photo & Video Release
I grant Old Pueblo Football permission to photograph and record my child at OPF events and to use those photos and videos, including photos families upload to OPF folders, on opfaz.com, social media and promotional materials, without compensation. This release is required to register. I may ask OPF to remove a specific post by emailing info@opfaz.com.
7. Parent/Guardian Supervision
Every participant under 18 must have a parent, legal guardian, or an adult (18 or older) approved by the parent or guardian present at the OPF event the entire time the participant is there, including before and after games. OPF, its coaches, officials and staff do not provide supervision or childcare. A participant without a parent, guardian or approved adult present may be held out of play until one arrives. I am responsible for my child's supervision, transportation and conduct at all OPF events.
8. Communications
I agree that OPF may contact me by email, text message and phone about registration, schedules, weather, safety and league news. Message and data rates may apply. Reply STOP to opt out of texts. Game-day and safety notices may still be sent by email.
9. Acknowledgment
I am the parent or legal guardian of the participant, or the participant is 18 or older. I have read these terms, understand that I am giving up substantial legal rights, and agree to them voluntarily. Checking the box below has the same effect as my written signature. These terms apply to the Winter 2027 season and any OPF event connected to it.
