In consideration of my/our child being allowed to participate in any way for the Pendleton Patriots/Pendleton County Youth Sports/NKYFL, its related events and activities, the undersigned, acknowledges, appreciates, and agrees that: 

1) The risk of injury from the activities involved in this program is significant, including the potential risk for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce the risk, the risk of serious injury does exist; and 

2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, 

3) I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe an unusual and significant hazard during my presence or participation, I will bring such to the attention of the nearest official immediately; and 

4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Pendleton Patriots, its officers, officials, agents and/or volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law. 

5) If a child leaves a practice or game for any medical reason, he or she cannot return without medical clearance from a physician. 

6) At any time, a coach can require medical clearance to be able to participate or play at his/her own discretion. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

PLEASE READ ENTIRELY!!       PENDLETON COUNTY YOUTH SPORTS PARENT CODE OF CONDUCT 

1. I will not force my child to participate in sports. Also, Coaches will not force your child to participateThey will encourage, but not force. 

2. I will remember that children participate to have fun and that the game is for youth, not adults. 

3. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others. 

4. I have read and understand that I must follow the PCYS and NKYFL by-laws which are found on their websites.    

5. I will convey the conduct policy to family members and friends and that I can be held accountable for those in my party. 

6. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice, or other sporting event. 

7. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures. 

8. I will represent and portray Pendleton County Youth Sports, all affiliated and the NKYFL in a positive light (social media, public venues, etc.) 

9. I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes. 

10. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence. 

11. I will demand that my child treat other players, coaches, officials, and spectators with respect regardless of race, creed, color, sex, or ability. 

12. I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the outcome of a game or his/her performance. 

13. I will never ridicule or yell at my child or other participants for making a mistake or losing a competition. 

14. I will respect the officials and their authority during games and will never question, discuss, or confront officials or coaches on the game field. 

15. I will abide by the 24-hour rule following games prior to speaking to or questioning a coach24 hours allows for emotions to settle and give time to think more clearly about any issues or concerns.   

16. As issues or concerns arise, I WILL address those issues directly with my child’s coach in a civil manner prior to bringing it to the attention of the Football Director, then the President or Board. 

17. I understand that a parent or other responsible party MUST be present for the entire duration of practices, games, and events that my child is participating in unless your Team Mom is notified in advance. 

18. I will remain in designated areas for parents during the events, (games, practices, etc.) I.E., not on sidelines or entering practice field without permission. 

19. I will demand a sports environment for my child that is free from drugs, and I will refrain from their use at all sports eventsTobacco products including Vapes are to be used in designated areas only and not in the stands. 

20. I will refrain from coaching my child or other players during games and practices unless I am one of the official coaches of the team. 

21. I understand that I can be asked to vacate the premises by PCYS Football Director, Executive Board Member or the NKYFL (Northern Kentucky Youth Football League) 

 I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but is not limited to the following: 

· Verbal warning by official or head of league organization 

· Suspension 

· Expulsion 

Suspensions and Expulsions will be made by the Pendelton County Youth Sports Executive Board or NKYFL Executive Board. Suspensions & Expulsions will have a right to appeal.
