Playmaker Basketball

By completing and signing this registration form, I confirm that I am the parent or legal guardian of the player named below and agree to the following terms on behalf of myself and the registered player.

I understand and agree that:

1. Registration & Information
   All information provided during registration is complete and accurate. I will notify Playmaker Basketball of any changes to my contact, emergency, medical, or other relevant information.
2. Program Commitment
   I understand that participation in a Playmaker Basketball program requires a reasonable commitment to practices, games, tournaments, training sessions, and other scheduled team activities. I will make every reasonable effort to ensure that my child arrives on time and prepared to participate.
3. Player Development & Playing Time
   I understand that Playmaker Basketball prioritizes player development, teamwork, effort, coachability, character, and long-term growth. Playing time, positions, starting roles, or specific team responsibilities are not guaranteed and are determined by the coaching staff based on player development, attendance, effort, coachability, team needs, game circumstances, and other appropriate factors.
4. Fees & Payments
   I agree to pay all applicable program fees, deposits, and other charges according to the payment schedule provided by Playmaker Basketball. I understand that additional costs may apply to activities or items not included in the advertised program fee.
5. Withdrawal & Refunds
   I understand that registration and team placement may require Playmaker Basketball to commit funds and resources in advance. Refunds are therefore subject to Playmaker Basketball's applicable refund policy. Costs already committed or paid, including league fees, tournament fees, uniforms, facility rentals, registration fees, and other non-refundable expenses, may not be refundable.
6. Schedule & Facility Changes
   I understand that practice, game, tournament, and facility schedules may change due to permits, facility availability, league scheduling, weather, closures, or other circumstances beyond Playmaker Basketball's control. Updated information may be communicated through TeamLinkt or other official Playmaker communication channels.
7. Player & Parent Conduct
   I agree that both the player and parent/guardian will follow Playmaker Basketball's Player & Parent Handbook, Code of Conduct, Safe Sport Policy, Concussion Policy, and other applicable policies and procedures. I understand that respectful behaviour toward players, coaches, referees, opponents, staff, facilities, and other families is required at all times.
8. Health & Safety
   I confirm that I will provide accurate information regarding my child's relevant medical conditions, allergies, injuries, medications, emergency contacts, and other information necessary for safe participation. I understand that my child should not participate when they are injured, ill, or otherwise unable to safely participate.
9. Emergency Medical Assistance
   In the event of an emergency involving my child, I authorize Playmaker Basketball coaches, staff, or representatives to take reasonable steps to respond to the emergency and, when reasonably necessary, contact emergency medical services or arrange for appropriate medical assistance if I cannot be reached in time.
10. Photography & Video
    I understand that photographs and video may be taken during Playmaker Basketball practices, games, tournaments, training sessions, and events. I authorize Playmaker Basketball to use approved photographs and video of my child for legitimate Playmaker purposes, including social media, website content, marketing, promotional materials, team communications, and printed materials.
11. Player Participation & Program Standards
    I understand that Playmaker Basketball may take appropriate action, including suspension or removal from a program, where a player or parent/guardian seriously or repeatedly violates Playmaker policies, conduct standards, payment obligations, team commitments, facility rules, or other program requirements.
12. Acknowledgement of Playmaker Policies
    I confirm that I have had the opportunity to review the applicable Playmaker Basketball policies and understand that those policies form part of this registration agreement.

Parent/Guardian Declaration

By signing below, I confirm that:

• I have read and understood this Player Registration Agreement & Waiver.
• I agree to all of the terms and conditions stated above.
• I agree to follow the applicable Playmaker Basketball policies and procedures.
• I understand that this agreement applies to both myself and the registered player.
• I understand that my electronic signature has the same intent as my written signature for the purpose of this registration.

• A $50 non-refundable administration fee will be deducted from any eligible program refund.
• No refunds, transfers, or program changes will be permitted within 14 days of the program start date.
• No prorated refunds or credits will be provided for sessions missed by the participant, regardless of the reason for absence.
• No individual make-up sessions or partial make-up sessions will be provided for participant absences.
• If a session is cancelled by Playmaker or the facility/gym due to circumstances beyond the participant’s control, Playmaker will make reasonable efforts to provide a make-up session at a later date, subject to facility and coach availability.
• If a make-up session cannot reasonably be scheduled, Playmaker will communicate the available options to affected families.

By registering, you acknowledge and agree to the above Refund & Make-Up Policy.

By registering with TeamLinkt, you commit to adhere to the refund policies, fees, and timelines set by your chosen organization or program. You commit not to initiate any chargebacks.  Deviations from this policy may result in the forfeiture of any refund.
