By submitting this form you are agreeing to the following;

 1.  I, the parent or guardian of the above player do hereby give my consent for the above player to play contact football and to participate in the activities of the RMF 2000 Inc.

2. I hereby release the RMF 2000 Inc. and every officer, director, sponsor, agent, and representative jointly and severally from any obligation, liability, costs, claims, and demands for damages and every claim and demand whatsoever for any personal or other injury, including death, or any loss, sickness or damage arising out of or in connection with the contact of any said persons in organizing, supervising or conducting their activities.

3. As a parent, if I choose to purchase a personal helmet for the above, I agree to have the helmet inspected by RMF at a cost of $20 on a date TBD before the evaluations starts. 

The fee to register for the RMF South Sask U16 is $100, which must be paid in full at the time of registration. Of this amount, $100 is a non-refundable registration fee that covers insurance and administrative costs. Players attend an evaluation, and will be notified if they will be advancing to the next evaluation.

The following rules apply to all of our programs:

• Player Restrictions: If registration numbers exceed capacity due to unforeseen restrictions (e.g., COVID protocols, spring camp limitations, or equipment shortages), players will be prioritized on a first-come, first-served basis. Priority will be given to those who have registered and paid.

• If 50% or more of the scheduled games are completed and the season is subsequently canceled or ends early, the season will be considered complete, and no refunds will be issued.
