Assumption of Risk, Waiver and Release of Liability

The undersigned, on my own behalf and as parent or guardian, and on behalf of, the minor registering for and participating in this football program, both the practices the subsequent games or activities operated by or participated in by the RMFL, acknowledges that football is an inherently dangerous sport in which injury of varying degrees of severity, up to and including death, can occur and that the intention of this document is to assume risk and waive and release claims of liability.

On my own behalf and on the behalf of the minor registering and participating in this football program and any such subsequent games or activities, I hereby waive and release any claim whatsoever against the RMFL, as well as any officers, directors, coaches, managers, trainers, volunteers associated with the RMFL for any claims, damages, injuries or losses to personal property and/or for personal injury or death arising out of the participation of the minor in this football program and any games or activities they participate in as a member of the football program. On my own behalf and on behalf of the said minor, I also agree not to bring any claim against a third party who may in turn claim against the RMFL, and/or its officers, directors, coaches, managers, trainers, volunteers associated with the RMFL, including for contribution or indemnity. I have read and understand this Assumption of Risk, Waiver and Release of Liability.

Parental Medical Release

I am satisfied that my child is in good health to take part in strenuous activities. He/she has my permission to participate in these approved physical activities, and I absolve the Rural Manitoba Football League and/or any of its staff, directors, executive, associations, members, volunteers, or sponsors from any and all liability whatsoever in relation to their participation in the sport. I also agree with the need to have my child examined by a physician following an illness or injury to re-establish a bill of good health, and that this or any other medical examination is my sole responsibility.

Photographic/Media Consent Form

I hereby consent to the collection and use of my child's personal images by photography or video recording. I acknowledge these may be used on the RMFL website, in newsletters and publications as well as distributed to members. I further acknowledge that my child's image may be used by the RMFL to promote football in the future.
