In consideration of Regina Youth Flag Football League and Football Saskatchewan is accepting the application of those you register to participate in flag football related events and activities, I / we, the parent(s) / guardian(s) of the players to be registered, hereby give my / our approval to his / her participation in any and all the activities and programs of the Regina Youth Flag Football League and Football Saskatchewan.

I / we agree to RELEASE, SAVE HARMLESS, AND INDEMNIFY Regina Youth Flag Football League, Football Saskatchewan, it’s Directors, Officers, Employees, Representatives, Sponsors, Coaches, and Agents from all claims, actions, costs, expenses, and demands in respect to death, injury, loss or damage to my son/daughter or our property whose soever and howsoever caused, arising out of or in conjunction with my child's participation. I further acknowledge that I am aware of the risks inherent in participating in the sport of flag football and I assume the risks and waive notice of all conditions, dangers and agree that this release shall bind my heir, executors, administrators and assigns.

I the Parent / Guardian of the applicant have read this release and agree to Indemnify Regina Youth Flag Football League and Football Saskatchewan, its servants, agents, or employees from any claims or demands, which might be made against the Regina Youth Flag Football League, Football Saskatchewan out of or in consequence or participation by the applicant. You also allow the league to use any photographs and/or videos taken to be displayed on the website, social media or on any league promotional materials. These will not be sold to any third party entities.
