I, the undersigned, certify the information provided in the registration to be true, and in consideration of being granted participation in the Sherwood Park Kings Athletic Club (SPKAC) hockey program, with the privileges incident thereto, acknowledge that by signing this certificate, all registered participants have become subject to the rules, regulations and decisions of SPKAC and it’s Board of Directors.  I acknowledge that these regulations and decisions may be restrictive in some areas such as movement from team to team, conduct etc., and I agree to abide by such rules, regulations and decisions of the SPKAC, its Board of Directors, its parent Associations, governing bodies and partnering leagues.

In consideration of the Sherwood Park Kings Athletic Club (SPKAC) allowing my son/daughter to participate in their hockey program, the undersigned being the lawful parents/guardians of the player named in this registration form, hereby releases and forever discharges SPKAC from all claims, demands, costs, expenses, damages, suits, general or consequential damages, and injuries, howsoever caused including and without limiting a generality of the foregoing, any negligence of the aforementioned persons, for or in respect to our/my child in the SPKAC hockey program or related activities of the SPKAC hockey program. 

The information requested in this registration form is required by SPKAC to facilitate hockey programs on behalf of the registrant, SPKAC. SPKAC will treat this personal information with the utmost respect and in accordance with the SPKAC Privacy Policy at all times. SPKAC does not sell, trade or otherwise share the information collected outside of Hockey Alberta, it’s minor hockey association, leagues or clubs. However, we may from time to time use this information for the purposes of offering promotions, additional services and/or hockey specific research.

CANCELLATION POLICY 

• Withdrawals/cancellations requested within one week of the tryout start date will result in the forfeit of the tryout fee, except under exceptional circumstances such as injury resulting in the player not being able to attend or complete tryouts. To receive a refund under this provision, the request must be made in writing to the Registration Director. Supporting documents, (i.e. doctor’s report) may be requested. 
• Players suspended or expelled for disciplinary reasons shall be ineligible for a refund of fees;
