By registering with Sherwood Park District Soccer Association (Phoenix FC) for any programs, leagues, or activities (the “Programs”), I, the participant (or parent/guardian if under 18), acknowledge and agree to the following:

1. Eligibility and Registration
- Registration is confirmed only when all required forms, waivers, and full payment are received.
- Participants must meet all age and eligibility requirements set by Phoenix FC.

2. Program Participation
- I consent to participate in all Program activities, including travel by private or public transport for games or practices outside Sherwood Park.
- I understand that registration fees cover league, academy, or camp costs as described for each program and do not include costs for tournaments or travel.
- I will advise the coach in writing of any medical conditions, medications, or other information necessary to safeguard my health.

3. Code of Conduct and Compliance
- I will follow all Phoenix FC rules, policies, and facility requirements.
- I will demonstrate good sportsmanship and respect for teammates, opponents, officials, and facilities.
- Abusive, discriminatory, or unsafe behaviour may result in suspension or removal without refund.

4. Assumption of Risk and Waiver of Liability
- I acknowledge that soccer and related activities involve inherent risks, including injury or property damage.
- I voluntarily assume all risks and release Phoenix FC, its staff, volunteers, and affiliates from any liability for injury, loss, or damage arising from participation, except where prohibited by law.
- I understand that insurance coverage is limited to the extent of my Alberta Soccer Association membership.

5. Fees and Refunds
- Initial payment of fees must be paid before participation.
- Refunds will follow Phoenix FC’s refund policy and may include deadlines or administrative fees.
- Missed sessions or participant-initiated withdrawals are not eligible for refund unless otherwise stated.

6. Privacy Consent
- I authorize the collection and use of personal information (e.g., name, date of birth, email, gender, city, postal code) for soccer administration and communications from Phoenix FC, Alberta Soccer Association, and Canada Soccer.
- Information is collected under Alberta’s Personal Information Protection Act (PIPA) and will not be sold or shared outside the organizations listed above.
Our Phoenix FC programs are run on the basis of fair play, enjoyment and kindness. Abilities and ages can vary in these leagues, therefore it is essential that all participants are aware that foul, abusive or overly aggressive play, language and other negative behaviours towards match officials, team officials, opponents, teammates and Phoenix FC representatives may greatly affect the spirit and culture of these leagues and will be addressed accordingly.
Sherwood Park District Soccer Association O/A Phoenix FC is committed to fostering respect, safety, and fair play. As a player, I pledge to uphold the following standards in alignment with Alberta Soccer Association guidelines.

1. Respect for the Game
• I will follow and play by the rules.
• I will not argue with referees or officials. Concerns will be shared with my coach in a respectful manner.
• I will control my temper and avoid abusive or provoking behaviour.

2. Effort and Teamwork
• I will work hard for myself and my team.
• I will cooperate with coaches and teammates.
• I will participate for my own enjoyment and development, not only to please others.

3. Sportsmanship
• I will be a good sport and respect opponents, teammates, and officials.
• I will treat all participants as I would like to be treated.
• I will applaud good play from both teams.

4. Communication and Online Conduct
• I will not use texting, email, or social media to harass or criticize others.
• I will report any online harassment to my coach, parents, or the club.

5. Equity, Diversity, and Inclusion
• I will respect all participants regardless of gender, ability, orientation, background, or religion.
• I will help create a welcoming and inclusive team environment.

6. Compliance
• I will comply with all Phoenix FC and Alberta Soccer Association rules and policies.


Player Accountability & Disciplinary Framework

Sherwood Park District Soccer Association O/A Phoenix FC expects all players to follow the Player Code of Conduct. Breaches will be addressed in alignment with Alberta Soccer Association and Edmonton Minor Soccer Association standards.

Level 1 – Minor Infractions
Examples:
• Occasional inappropriate language.
• Showing disrespect through body language.
• Failure to follow instructions.

Potential Consequences:
1. Verbal warning and discussion.
2. Written warning and notification to parents.
3. Temporary suspension (1 game or session).

Level 2 – Moderate Infractions
Examples:
• Repeated misconduct after warnings.
• Disrespecting officials or opponents.
• Inappropriate online behaviour.

Potential Consequences:
1. Formal written warning and parent meeting.
2. Suspension (2–4 games/sessions).
3. Probationary period with behaviour plan.

Level 3 – Major Infractions (Zero Tolerance)
Examples:
• Bullying, abuse, or harassment.
• Violence or threats.
• Substance use while participating.
• Criminal behaviour.

Potential Consequences:
1. Immediate suspension pending investigation.
2. Removal from team/program.
3. Mandatory reporting to ASA/EMSA or law enforcement where applicable.
Sherwood Park District Soccer Association O/A Phoenix FC believes that parents and guardians play a critical role in supporting a positive soccer environment. Parents are expected to model respect, fairness, and encouragement in alignment with Alberta Soccer Association guidelines.

1. Supporting My Child
• I will remember that my child participates for their enjoyment, not mine.
• I will not force my child to play.
• I will teach my child to play by the rules and resolve conflicts without hostility.

2. Encouragement and Respect
• I will encourage effort and doing one’s best over winning.
• I will never ridicule or yell at my child or another player.
• I will applaud good sportsmanship and effort by all players.

3. Respecting Officials and Coaches
• I will respect officials’ decisions and teach my child to do the same.
• I will support all efforts to remove verbal and physical abuse from children’s sport.
• I will respect and show appreciation for coaches and volunteers.

4. Communication and Online Conduct
• I will not use texting, email, or social media to harass or criticize others.
• I will report any incidents of online misconduct to the club.

5. Equity, Diversity, and Inclusion
• I will respect all participants regardless of gender, ability, orientation, background, or religion.
• I will contribute to a welcoming environment for all.

6. Compliance
• I will comply with all Phoenix FC and Alberta Soccer Association rules and policies.


Parent Accountability & Disciplinary Framework

Sherwood Park District Soccer Association O/A Phoenix FC expects all parents to follow the Parent Code of Conduct. Breaches will be addressed in alignment with Alberta Soccer Association and Edmonton Minor Soccer Association standards.

• Questioning referees in an inappropriate manner.

Potential Consequences:
1. Verbal warning.
2. Written warning.
3. Removal from game or session.

Level 2 – Moderate Infractions
Examples:

• Harassment of coaches, players, or officials.
• Inappropriate use of social media.

Potential Consequences:
1. Formal written warning and meeting with Executive Director.
2. Suspension of spectator privileges for 2–4 games.
3. Probationary status for the season.

Level 3 – Major Infractions (Zero Tolerance)
Examples:
• Threats, abuse, or violence toward any participant.
• Harassment or discrimination of any kind.
• Criminal behaviour.

Potential Consequences:
1. Immediate removal from the facility.
2. Removal of family from the program.
3. Mandatory reporting to ASA/EMSA or law enforcement where applicable.

Refund Policy for Additional Programs

This section applies to: Training Camps (SPARK, PD Day, Summer Camps, etc.)

Phoenix FC is a non-profit organization committed to providing high-quality soccer programming with flexibility. This refund policy helps protect the club from last-minute player withdrawals in its additional programs. It also accounts for credit card processing fees on transactions.

  • Player-Initiated Withdrawal: Withdrawals from the program that are initiated by the player will be reviewed and a registration credit will be issued to the registrant for future use.
    • Note: Extenuating circumstances will be reviewed on a case-by-case basis to determine if a prorated refund is appropriate.
  • Club-Initiated Cancellation: Cancellations of the program(s) that are initiated by the club will result in a full refund.
    • Note: In the event of a cancellation, registrants may be offered a registered place in a similar program in lieu of a refund.
