Terms of Use:

As the parents/guardians of the child I hereby consent to his/her participation in the Yorkton Basketball Camp, sponsored by Saskatchewan Youth Basketball. I/we do hereby release Saskatchewan Youth Basketball, coaches referees, volunteers, and/or employees, other players or participants, owners and leasers of premises used to conduct the program from all liability in respect to any injury sustained by my son/daughter while participating in the camp.
I understand that program fees include membership in Basketball Saskatchewan (BSI) which provides liability and sport accident insurance and my contact information will be shared with BSI. For more information see www.basketballsask.com under membership.
I hereby authorize the reproduction, copyright, exhibition, broadcast and/or distribution of any Saskatchewan Youth Basketball practice or tournament still or video photography without limitation.
I consent to the use of my email address in order to receive communication from Saskatchewan Youth Basketball.
I understand the following refund policy. Registration fees will be refunded in full if the camp is cancelled. No other refunds will be provided.
