ADULT WAIVER/CONSENT FOR MINOR PARTICIPANT RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT BY SIGNING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY! In consideration of being allowed to participate in any way in the SMASH Volleyball program and any SMASH Volleyball affiliated club programs including but not limited to SMASH Volleyball, related events and activities, the undersigned hereby acknowledges and agrees that: 1) I am the parent/guardian having full legal responsibility for decisions regarding my child/ ward; 2) I am familiar with and accept, on behalf of my minor child/ward that the risk of injury from the activities involved in this program is significant, and while particular rules, equipment, and personal discipline may reduce this, the risk of serious injury does exist; 3) I have satisfied myself and believe that my minor child/ward is physically, emotionally and mentally able to participate in this event, and that his/her equipment, is mechanically fit for his/her use in this event; 4) I understand, and will instruct my minor child/ward, that all applicable rules for participation must be followed and that at all times the sole responsibility for personal safety remains with my minor child/ward; 5) I will immediately remove my minor child/ward from participation, and notify the nearest official, if at any time I sense or observe any unusual hazard or unsafe condition or if I feel that my minor child/ward has experienced any deterioration in his/her physical, emotional or mental fitness for continued participation in the event; 6) By signing below, I authorize the publication of any photography taken for or during this program for the use of promoting or advertising further programs, unless I notify SMASH Volleyball, of my desire to not permit any published photos at the time of registration; and 7) By signing below, I acknowledge that I have read, I understand, and I agree to the SMASH Volleyball Conduct and Policy form. By signing below, I acknowledge that if policies stated therein are abused or broken, it can result in my child/ward’s suspension from SMASH Volleyball programs, with or without prior warning. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS SMASH Volleyball their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event ('Releases'), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.


In order to request a refund, a written email should be sent to smashvolleyballnl@gmail.com with a detailed explanation for the request. After the request has been received, our board will review it. If the request is approved, you will be contacted by our executive staff. If a refund is approved, there will be a $25.00 non-refundable administration fee.
