If you reside within the SMHA’s boundaries, and have missed your home zone’s main and late on-line registration deadlines, please complete this online form in its entirety and submit it to SMHA.

Please note that players who apply with SMHA to register after August 20, 2024 will be placed on a first-come-first-placed basis.
Players are not guaranteed to be registered into their home zone, or placement on any particular team.
Placement will be based on registration numbers and the requirements throughout the SMHA zones and may occur once league play has begun.
Payment of registration fees will need to be completed directly with the receiving zone before the player will be allowed to be on the ice.

Terms and Conditions

For late registration applications:

SMHA cannot guarantee that a player will be able to register with their home zone. All efforts will be made to place the player into their home zone but registration numbers in each zone will dictate where players will be placed.
SMHA cannot guarantee that a player will be able to register for the current season. If an age group is full across all zones, a player will not be able to be registered.
Late registration applicants must pay their registration fees to the zone they are placed in prior to being placed onto a team or being permitted to step on the ice. Note:Registrations after June 30th are subject to a $100 late surcharge. Exceptions: players new to SMHA and players in the U7 age group.
For future seasons, please keep in mind that main online registration takes place June 1-30 each season with late online registration open from July 1 - August 20. A late fee is applicable in most cases after June 30th.

The SMHA Zone that your player registers with will have a published refund policy that will be applicable to you. Once you know which zone you will be registering with, please refer to that Zone's refund policy or contact the Zone Treasurer or Registrar for more information.
