Sherwood Park Minor Hockey

I, the undersigned, certify the information provided in the registration to be true, and in consideration of being granted participation in a SPMHA hockey program, with the privileges incident thereto, acknowledge that by signing this certificate, all registered participants have become subject to the rules, regulations and decisions of SPMHA (hereinafter referred to as the 'Association') and it’s Board of Directors.  I acknowledge that these regulations and decisions may be restrictive in some areas such as movement from team to team, conduct etc., and I agree to abide by such rules, regulations and decisions of the Association, its Board of Directors, its Minor Hockey Associations, Leagues, or Clubs. 

Further, the contact information requested in the registration process is required by the Association to facilitate hockey programs on behalf of the registrant and the Association. The Association will treat this personal information with the utmost respect and in accordance with the Association Privacy Policy at all times. SPMHA does not sell, trade or otherwise share the information we collect outside of the Association, though the Association may use this information to contact you from time to time with information relevant to our hockey program on behalf of itself or it's partners.  In accordance with Canadian Anti-Spam Legislation, you will have the ability to unsubscribe from these communications at any time.

I also understand that Sherwood Park Minor Hockey Association is not responsible for any matter, cause or accident which may occur during the period of this regulation.  I accept this as a condition of registration.

SPMHA CAMP REFUND POLICY

PRIOR TO A PROGRAM STARTING

Up to one week prior to start of program/camp:

Full refund minus administrative fee of $20 and any online service charges.

From 1 week to 72 hours prior to start of program/camp:

Full refund minus administrative fee of $50 and any online service charges.

From 72 hours prior to start of program/camp:

No refund

DURING A PROGRAM

If a refund is deemed appropriate, it will be pro-rated to the amount of time the participant was in the program. For example: A camp costs $235 for five one-hour sessions and a participant is injured in session two, and unable to complete the camp, the participant shall be entitled to a $141 refund.

SPMHA CANCELLATION

In the event that SPMHA cancels an camp session, participants will get a full refund for any sessions cancelled.

OTHER

In individual cases involving extenuating circumstances that fall outside of this policy, a refund request must be submitted in writing by the participant to SPMHA. Each request will be reviewed by the SPMHA General Manager, who will make the decision as to whether a refund will occur.
