Participation Acknowledgement:
As the parent or legal guardian, I understand that my child will participate in soccer training and games organized by Sportify. I acknowledge the risks involved, including injury, illness, or death, and I understand that Sportify is not liable for any of these risks.
Medical Authorization:
I certify that my child is physically fit for soccer training and has no medical conditions that would prevent participation. In case of emergencies, I authorize Sportify and its representatives to seek medical attention for my child on my behalf.
Transportation Responsibility:
I understand that it is my responsibility as the parent or guardian to arrange for transportation to and from all practices, games, and related events. I acknowledge that Sportify does not provide transportation services for players.
Supervision Acknowledgment:
I understand that Sportify is not responsible for supervising my child before or after practices and games, including pregame warm-ups and related events. It is my responsibility as the parent or guardian to ensure that my child arrives at the designated practice or game location on time and is picked up promptly after.
Emergency Contact Information:
I agree to provide accurate and up-to-date emergency contact information to Sportify, including phone numbers where I can be reached in case of an emergency during practices or games.
Photo and Video Release:
I grant Sportify permission to use photographs and/or videos of my child for promotional or educational purposes, releasing the organization from any liability associated with their use. I understand there is no time limit on the validity of this release, no compensation to me or my child, and no control over the further distribution or use of these materials.
Cancellation and Rescheduling:
In the event of practice or game cancellations due to inclement weather or unforeseen circumstances beyond our control, Sportify reserves the right to cancel and reschedule sessions, endeavoring to ensure that participants receive the benefit of their training as best as possible. Make-up times may NOT always be guaranteed. Note that no refunds will be given for canceled sessions.
Release of Liability:
I agree to release, indemnify, and hold harmless Sportify, its staff (directors, officers, employees, and agents), volunteers, and representatives (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, liabilities, or expenses, including attorneys’ fees, arising out of or in connection with my child’s participation in the soccer training, whether caused by the negligence of the Released Parties or otherwise.
CONFIRMATION OF AGREEMENT AND OF UNDERSTANDING:
BY EXECUTING THIS SUBMISSION, I AFFIRM THAT I AM THE PARENT OR LEGAL GUARDIAN OF THE AFOREMENTIONED MINOR AND POSSESS THE REQUISITE LEGAL AUTHORITY TO EXECUTE THIS WAIVER AND RELEASE OF LIABILITY. I HAVE READ, UNDERSTOOD, AND AGREE TO THE TERMS AND CONDITIONS SET FORTH IN THIS DOCUMENT.
